1852608 Primary Facility 2009-12-28 Routine/Complete Inspection No Violations Noted According to record, operator completed the well on 9-9-08 to depth of 3818 ft. In reviewing well record, the well was "permanently cemented" with the proper calculated volume of cement to fill the annular space plus 20 %. In addition, the well head pressure is currently 600 psi (according to inactive application while site gauge showed 720 psi). The gas wellhead shut-in pressure in both cases does not exceed the 0.433 psi/foot of surface protective casing depth. Upon additional inspection of the well, the well head has been not properly well tagged. DEP could locate well tag at the wellhead. No signs of odors or leaks occurred around the well head. DEP called McIntire to confirm well head pressure and install/locate the permanent well tag.
1897055 Primary Facility 2010-06-29 Site Restoration No Violations Noted At time of inspection, well was drilled but not yet equipped for production. Inactive statues granted as of 1/19/10. Well was properly tagged. Site was adequately and uniformly restored with >70% vegetative cover. Access road was stable. No accelerated erosion or sedimentation issues were noted in relation to well site or access road. (During inspection accompanied with WQS Greg Higgins and Jarrod Shultz).
