1814873 Primary Facility 2009-07-22 Drilling/Alteration No Violations Noted Currently, well completed to a depth of 1,010 ft. below grade. Well site was identified with well sign. However, name of well was correct but permit number did not match site number. Slight visible E&S concerns at the time of inspection with sediment occurring on the road and no filter fence surrounding the pad site (not included in E&S plan). Requested Cambria DEP water quality personnel to perform an evaluation. No IW discharge issues. Pit aligned with 20 mil plastic and had no freeboard issues. Currently, 13' 3/8" conducter set at 33 ft., 9 5/8" pipe set at 252 ft., 7" set at 969 ft. Cement - 75 sks for 9 5/8", 7" 175 sks. Each cement had return BBl of 9 and 12 respectively. Coal seems observed at 365 ft, 440 ft, and 520 ft. Water located at 60 ft and 540 ft. Complete well log present at the site.
1815553 Primary Facility 2009-07-24 Routine/Complete Inspection No Violations Noted Performed routine inspection to evalulate the need for addition E&S controls. It was determined from my inspection that the terrain has a slope of approximately 3% and is located in a very well vegetated area. Therefore the vegetative cover should be sufficient to prevent erosion and sedimentation. Discussed the site with Chad Wagner and the access road entrance. The operator should continue to monitor the highway for any mud that may be a hazard to traffic, the access road, and well site for erosion and sedimentation. Well has been drilled, but not in production.
1888995 Primary Facility 2010-06-03 Site Restoration No Violations Noted At the time of inspection, the well was drilled and in production. Well labeled on data logger. Site was adequately and uniformly restored. No accelerated erosion or sedimentation issues were noted. Brine tank appeared sound. Access road was in good condition. It is recommended that the operator promptly remove a used methanol barrel that was left onsite.
