1917374 Primary Facility 2010-09-30 Drilling/Alteration No Violations Noted According to records, well sign and E&S plan was located along access road. No E&S issues and no IW issues during inspection. Heavy rains during inspection. First Energy was contracted to install well from Dorso. First completed and was pulling equipment out of site. First had a snow plow truck scrapping mud off road. Well completed with Conductor pipe consisted of 13 3/8¿ (sanded) to 42 ft. Water String was 9 5/8¿ to 294 ft with 120 sks of Class A. Surface casing (7¿) was installed to depth of 969 ft. and cemented with Class A of 175 sacks. Production string (4 ½¿) to Anticipated depth of 3660 ft with 44 sks of T177. Pit was covered with 20 mil plastic with security fence. No freeboard issue.
1919885 Primary Facility 2010-10-07 Routine/Complete Inspection No Violations Noted During this inspection, well site properly signed with ES plan onsite. Well drilled not yet equipped for production. Noted the proper installation and functioning of filter fence below well pad. One below and one above grade pit onsite, both sustained two of lined freeboard and appeared structurally sound. Spoke with Chad Wagner of Dorso via telephone, and noted possible concerns with access road entrance (recommended additional rock be added to reduce mud on roadway), and also noted what appeared to be a small diesel leak (<5gals) from vehicle near well head. Chad assured me he would be onsite later today, as he was currently in area. Chad also told me that plans were to backfill site on Monday 10/11/10, weather pending. A follow up inspection will be conducted to ensure possible pollution concerns are addressed
1968438 Primary Facility 2011-04-26 Routine/Complete Inspection No Violations Noted At the time of inspection, well Id located on brine tank. Well drill and in production. Site has been re-graded and seeded/mulched with minimal vegetative growth observed. Hence minimal growth observed, it is recommended that operator reseed and mulch to ensure uniform vegetative cover is established and sustained at minimum of 70%. Until final restoration activities are completed operator should continue to monitor/maintain all temporary BMP¿s in order to prevent a possible pollution event. Access road stable. No accelerated erosion/sedimentation or industrial waste issues observed onsite during this inspection. A follow up inspection will be conducted in the near future to ensure final, permanent site restoration is achieved.
