At the time of the inspection, no E&S concerns and no freeboard at the pit were present at the site. Filter fence was located on down gradient side of site. Phillips was contracted by NTS. Conductor pipe (20¿ pipe) was set at 30 feet and was sanded in. Mine void was encountered at 82 feet to 86 feet interval. Mine casing (13 3/8¿ pipe) was set at 116 feet with cement basket at 42 feet. 100 sacks of Type 1 were used to cement mine casing with return to surface. Plug was down at 9:35 pm at 1/4/12. Phillips was currently drilling depth of 178 feet. The anticipated DTD was 3,800 feet.

2376009 Primary Facility 2015-06-01 Follow-up Inspection Outstanding Violations - No Viols Req'd On-site inspection conducted as follow-up to inspection record 2353851 (enf ID 321127) for failure to install well tag in a legible, visible, and permanent manner and for failure to submit a well site restoration report.At time of inspection,well tag present, legible.Access road is developing ruts, therefore the Department recommends the Operator continue to monitor and maintain the access road and any associated BMPs (i.e. culverts) as necessary in order to prevent any potential pollution events from occurring. Well-tender appeared to utilize access road during the Department’s inspection. To date, the well site restoration report has not yet been received by the Department.Violations issued under 3211(h) for failure to install the well tag on the well in a legible, visible, and permanent manner have been corrected. 2376132 Primary Facility 2015-06-03 Administrative/File Review Outstanding Violations - No Viols Req'd Admin Inspection in relation to inspection conducted on 6-1-15 with WQS S. Foulk. Department called Operator Steve Phillips by phone on 6-3-15 and left a message. Mr. Phillips returned call the same day. Department recommended removal of temporary BMP’s; recommended maintaining culvert; stated that access road was developing ruts; and that site restoration report was not submitted. Mr. Phillips responded that he would take care of all issues in a timely manner and would call when they were addressed. He remarked that he did not use the top section of the access road, but entered the site by the bottom road. Mr. Phillips inquired about well tag and Department responded that the well tab was present, legible and permanenly attached and in compliance. Outstanding violations still exist for this site (reference Inspection #2353851). Department stated that a follow-up inspection would be conducted in approximately one week.
2383905 Primary Facility 2015-06-29 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspection in response to follow-up phone call on 6-29-15. Department contacted Operator by phone and recommended Operator remove and dispose of uninstalled temporary BMP’s on site, remove temporary BMP stakes and maintain culvert. Department recommended maintaining access road. Operator stated he was waiting on a ‘mini-excavator’ to aid in removal of BMP’s and it should be on site soon. Department stated a follow-up inspection will be conducted in about 10 days. Previously issued violations are outstanding for this site.

Department contacted Operator by phone on 8-4-15 in regards to follow-up site inspection and previous inspections #2383909, 2383905 and 2376132. Discussed removal and disposal of temporary BMP’s, and maintenance of culvert and access road. Operator stated he would call when recommendations have been met. Department stated it would inspect site on 8-7-15 and issue violations if warranted. Follow-up inspection will be conducted on 8-7-15 or 8-10-15.
