2490481 Primary Facility 2016-06-03 Routine/Complete Inspection No Violations Noted Arrived @ 14:00. Weather conditions at the time of the inspection were cloudy. No company representatives were present during the inspection. There was one well head shut-in, and painted red, no signage. The well head was approximately 300’ from the nearest residence. There was also a red marker post near the well head (see photo). An Altair 5X multigas meter (calibrated on 5/23/16) gave field readings of 3% methane near the ground in the annular space between the broken casing and the well head (see photo). Although the requirement for site or well identification for this well, and the gas leak are currently being evaluated, no violations were noted at the time of the inspection. Left location @ 14:15.
