2488439 Primary Facility 2016-05-23 Routine/Complete Inspection No Violations Noted Arrived at 10:15. Weather conditions at the time of the inspection were sunny and warm, following rain. No company representatives were present during the inspection. One well has been drilled. No site ID sign, well tag, or identification of any kind was observed at the site during the inspection. The well was located in a junk yard. Three wooden pallets were angled against each other and over the well head. The well head was painted bright yellow. The 9-5/8” casing was inside the 13”. The 9-5/8” moved freely within the 13”. Water was visible in the 13” approximately 1.5’ down from the top of the casing. There was no bubbling visible. The well did not appear to be capable of production at the time of the inspection. An Altair 5X multigas meter (calibrated on 05/23/16) was used to monitor for the presence of combustible gas at well cap. No combustible gas was detected at the time of the inspection.
