2738429 Primary Facility 2018-06-05 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection with Steve Melman of DCNR, and Larry Fetterman and Drew Adamo of Diversified Gas & Oil Corporation (DGOC) on June 5, 2018.

DGOC is planning to plug conventional well 027-20901 (COP 706-8) starting in June-July 2018. This well is located on Sproul State Forest. This well is owned/operated by Alliance Petroleum Corporation (APC) which is a subsidiary of DGOC. Department records indicate that this well is still permitted to Mid East Oil Company and not APC. Mr. Adamo has provided me with transfer of well permit documentation dated June 2016. The Department has approved the transfer but it appears that this transfer was an over sight within the Departments efacts system. This information will be submitted to the Southwest District office to make the correction.

As discussed during the field meeting, the plugging rig will be placed on containment. In addition, any tanks or fueling tanks shall be placed on secondary containment. A well c

2748218 Primary Facility 2018-07-03 Plugging(Includes Plugged/Mined Through) No Violations Noted Howard Drilling are off location following plug and abandonment. At the time of inspection, recoverable 4 1/2" is out of the hole. 8 5/8" surface casing is open, no visible or audible venting. No gas was detected with an Altair 5x gas meter. The plugging contractor intends to top off with gravel and install marker pipe. No obvious issues were observed.
