2738251 Primary Facility 2018-06-05 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection with Steve Melman of DCNR, and Larry Fetterman and Drew Adamo of Diversified Gas & Oil Corporation (DGOC) on June 5, 2018.

DGOC is planning to plug conventional well 027-21270 (COP 706-11) starting in June-July 2018. This well is located on Sproul State Forest. This well is owned/operated by Alliance Petroleum Corporation which is a subsidiary of DGOC. The "notice of intention by well operator to plug a well" has already been submitted and received by the Department.

Please note the production equipment is labeled with well permit number 027-21075. This well permit was issued to Mid East Oil Co. and the operator reported "Not Drilled". This permit expired on 1/10/04. A new well permit 027-21270 was issued on 3/11/04 in the same location and is currently active.

As discussed during the field meeting, the plugging rig will be placed on containment. In addition, any tanks or fueling tanks shall be placed on secondary containment. A we

At the time of inspection; the plugged well is covered with gravel and a 2" non-venting monument with marker is installed at surface. No gas was detected with an Altair 5x gas meter. There were no open holes, discharges, venting, or other obvious issues at surface.
