Site has been constructed with large berm surrounding pad so that waters will be contained on site. Berm has been seeded and mulched, vegetation is about 50% on disturbed areas. Filter sock is inplace around berm. There are no E&S issues on site at this time. BMPs are installed and working properly. E&S plan is not on site, Mr. Jones called Mike Murphy of EOG and found that E&S plan has been stolen with mailbox and will be replaced shortly. Pit is constructed properly. Liner is shows no signs of tears or leaks. 2+ft of freeboard is being maintained. Water in Pit is mostly rain water and is being circulated from pit to hole to keep dust down.

2574690 Primary Facility 2017-03-23 Routine/Complete Inspection No Violations Noted Routine inspection of Anadarko COP Tract 231 on March 21, 2017. No one was on site during my inspection. The pad and surrounding the pad were covered in snow. There are six Marcellus wells located on this pad. Three of the six are producing. Wells 027- 21532 and 027–21549 are plugged. Well 027-21530 is in regulatory inactive status. All wells are labeled with the API and well permit numbers along with the operators contact information. There are three production units with metering systems online. There are four (400) BBL production water tanks located in secondary containment. The secondary containment was frozen with water. There appears to be no signs of leakage or failure to the containment.

In addition, there is a small compressor station on site. Connected to the compressor station is a 520 gallon waste oil tank that is double walled. In addition, a 1000 gallon double wall engine oil tank is connected to the compressor station.

2671831 Primary Facility 2017-12-15 Routine/Complete Inspection No Violations Noted The weather is 15 Deg. F and cloudy, windy at time of inspection.

Routine inspection of Alta Resources, LLC, C.O.P. Tract 231 A, Well # 1000H on December 15, 2017. There are 6 wells on location, 3 are producing.

Well is shut in and placed on inactive list. 9-5/8¿x 13-3/8¿ annulus is closed. 0% combustible gas was detected at the annulus vent or in the area immediately surrounding the well. 2681850 Primary Facility 2018-01-11 Routine/Complete Inspection No Violations Noted Routine site inspection at (Alta) ARD Operating, LLC COP Tract 231 Pad A on January 11, 2018. No one was onsite during my inspection.

There are six drilled Marcellus wells located on this well pad. The following 3 are producing: 027-21533, 027-21534, and 027-21548. Wells 027-21532 and 027-21549 are plugged and are represented by a labeled monument. Well 027-21530 is in regulatory inactive status. All wells are labeled with the API number and well permit number. Attached to the wells is a 325 gallon tote of foam stationed in secondary containment. There are 3 gas production units onsite connected to the producing wells. In addition, there are (4) 400 bbl produced fluids tanks stationed in secondary containment. Water within this containment was frozen and snow covered. One compressor unit is located onsite. There is a dehydration unit stationed onsite near the pad entrance.

2771956 Primary Facility 2018-07-25 Routine/Complete Inspection No Violations Noted The weather is 70 Deg. F and cloudy at the time of inspection. Routine inspection of Alta Resources, LLC., C.O.P. TRACT 231 A Pad, Well #1000H on July 25, 2018. This well is in Regulatory Inactive Status. Upon arrival to location well was shut in. Annulus vent (9-5/8" x 5-1/2") was closed. During the inspection verified pressures: annulus 475 PSI, backside casing - 847PSI, tubing - 845 PSI. 0% combustible gas was detected in the area immediately surrounding the well.
