Both well pads are covered under the same ESCGP. The ESCGP 1 has been upgraded to an ESCGP 2 and PCSMs have been installed. In addition, the previous operator Anadarko has transferred the ESCGP to ARD but any plug wells listed below have not been transferred from Anadarko to ARD.

COP Tract 259 Pad B: There are two drilled wells (027-21529 and 027-21567) located on this well pad. Both wells are represented with a tree on the well head. Both wells are in regulatory inactive status.

The site sign with emergency contact information, well permits, weekly E&S inspection reports, and emergency response plan are located onsite. The pad remains its original footprint size 400' x 400' and aggregate remains present throughout the pad. The topsoil stock pile is well vegetated. There are 2 inactive and shut in wells on this pad. Annulus is closed and not vented at time of inspection. Mechanical gauge on the production tubing reads pressure ¿ 5,000 PSI. 0% combustible gas was detected at the annulus or in the area immediately surrounding the well. All the production equipment has been removed from location. Observed gas bubbling in the cellar. Not able to monitor the level of gas leak due to the depth of the leak source in the cellar. There are 2 inactive and shut in wells on this pad. Annulus vent surface by intermediate (13-3/8¿ x 9-5/8¿) is closed and not vented at time of inspection; intermediate by production (7¿ x 4-1/2¿) is open and vented, 100% LEL and 22% combustible gas was detected. Mechanical gauge on the production tubing reads pressure 5,000 PSI. Observed minor gas bubbling in the cellar. Not able to monitor the level of gas leak due to the depth of the leak source in the cellar. No violations noted, but discussed follow up inspection with a company representative R. 2752808 Primary Facility 2018-03-27 Follow-up Inspection No Violations Noted Follow up inspection of Alta Resources, LLC., C.O.P. TRACT 259-B Pad, Well #1003 on March 27, 2018 due to gas bubbling in the cellar.

There are 2 inactive and shut in wells on this pad. Company operators pumped water out of the well cellar #1003 into well cellar #1001. During this process operators were able pin-point gas leak source. DEP recommendation for the operator was to collect a gas sample from the leak source. No violations noted, but discussed follow up inspection with a company representative Mr. Rick Hall.

Company operator opened annulus vent during inspection, 100% LEL and 10% combustible gas was detected. Vent was closed back in. Mechanical gauge on the production tubing reads pressure 5,052 PSI. No violations are noted at time of inspection.

2843527 Primary Facility 2019-02-07 Routine/Complete Inspection No Violations Noted The weather is 45 Deg. F and cloudy at the time of inspection. Routine inspection of Alta Resources, LLC., COP Tract 259 Pad, Well B-1003H, on February 7, 2019. There are 2 inactive wells on this pad.

This well is in Regulatory Inactive Status at the time of inspection, status expires on August 21, 2019. Production by intermediate annulus vent (4-1/2 X 9-5/8) is open and vented to the atmosphere. 0% combustible gas was detected at the annulus vent. Intermediate by surface (9-5/8" X 13-3/8") is open, vented to the atmosphere. Unable to use a meter due to annulus vent being covered with water in the cellar. Verified following pressures during inspection: production tubing is 5000 PSI, backside production casing pressure is 5043 PSI. 0% combustible gas was detected in the area immediately surrounding the well.
