There is one shut in well on this pad. Annulus is closed and not vented at time of inspection. 0% combustible gas was detected at the annulus or in the area immediately surrounding the well. 2839288 Primary Facility 2019-02-07 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at LPR Energy LLC Snow Shoe Well Pad 4 on February 7, 2019. No one was onsite during the time of the inspection. Multiple owners have owned this asset prior to LPR purchasing it.

During the inspection, the freshwater impoundment (FWI) was observed with approximately a 1-2 foot freeboard. The FWI is surrounded by a locked fence. LPR submitted a FWI discharge plan to the Department in June 2018 and had a contractor perform a freshwater discharge in June 2018. The Department recommends conducting another FWI discharge; weather permitting allowing at least a 2 foot freeboard. If the FWI discharge is performed after June 2019, then the operator should collect new water samples and submit those results along with an updated FWI discharge plan to the Department prior to discharging.

The Marcellus well pad has one well drilled on it (027-21672). According to efacts records this well was TD to a TMD of 13,565 ft and completed on July, 14,
