There is 1 non-producing well on this pad: (Permit: 37-027-21692). The well was shut in at time of inspection. Company operator projects to Plug and Abandon this well in 2018, do not have the date set for the event yet. Both annulus (13-3/8¿ and 9-5/8¿) are open and vented at time of inspection. 0% combustible gas was detected at the annulus vents or in the area immediately surrounding the well. No violations noted. 2731029 Primary Facility 2018-05-17 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection of this unconventional well was conducted May 17, 2018. I arrived on location prior to wireline. The well had a master valve and night cap. There was reportedly ~3900 psi on the production casing. There was an annular vent for the backside of production casing plumbed to surface. No flow or pressure was reported or observed on this annulus when operator opened the valve. The cellar was full of water below the casing head and no bubbling was observed. Wireline arrived on location, rigged up lubricator, and is preparing to run gauge ring then bridge plug to isolate lateral and pressure before the rig moves in to pump cement from the bridge plug back to surface. The production casing was cemented to surface and therefore unrecoverable, however the operator intends to perforate several spots in the 5 1/2" production casing where logging has indicated ratty cement to allow communication with the backside when pumping cement. No obvious issues were observed.
2732182 Primary Facility 2018-05-21 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection of this unconventional well was conducted May 21, 2018. A rig is on location for plug and abandonment. I met with the Companyman to discuss operations on the pad. The CIBP has been set with cement dump bailed on top to isolate the lateral. The well reportedly blew down with no problem thereafter. They are waiting on wireline for perforating. The operator has elected to perforate production casing in areas below the intermediate casing shoe where a CBL indicated ratty cement, to allow for communication with the annulus when pumping cement slurry. No obvious issues were observed or reported.
2735761 Primary Facility 2018-05-30 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this plugged unconventional well was conducted May 30, 2018. The rig is off location following P&A. The well has been cemented to surface following setting a CIBP in the production casing to isolate the lateral. At the time of inspection; the well has a 2" candy cane vent pipe with marker erected as a well monument. There was no visible or audible venting and no gas was detected at the vent pipe. No obvious issues were observed.
2765971 Primary Facility 2018-08-16 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of this recently plugged unconventional well was conducted August 16, 2018. The well is venting at a 2" candy cane vent pipe monument. A low volume of gas was observed venting at the time of this inspection.

At the time of inspection, visible and audible venting was observed at the 2" vent pipe monument. A field reading with an Altair 5x gas meter shows approximately 50% methane at the mouth of the vent and 100 % methane inside the vent. The well is in violation of 25 PA Code Chapter 78.91(a) for failure to plug the well to stop the vertical flow of gas within the well bore. See response requirements below.
