2603898 Primary Facility 2017-06-12 Incident- Response to Accident or Event No Violations Noted I arrived onsite @ 10:40 hrs. with Sue Herzing, WQS for Centre County to follow up on a spill reported by LPR @ 10:45 hrs. on June 8th, 2017. Clayton Hill of LPR left a voicemail with Ms. Herzing reporting that on June 7th, a water truck was pulling produced water from the tanks on the pad, and that he must not have closed the valve on the truck. A well tender discovered the release on the morning of June 8th. The volume was later calculated by LPR to have been 27 Bbls. of produced water released to the pad surface. Bigler Boyz was contracted to perform the excavation. Ms. Herzing and I were to meet representatives of Bigler Boyz and LPR on the site. The excavation area ran from the production tank containment to approximately 40' onto the pad, and ran over to the access road, narrowing down to approximately 10' as it went approximately 70' down the left side of the access road. Spots within the excavation were marked with dots and circles in red paint. No one else was present when we first arrived,
2678611 Primary Facility 2018-01-03 Routine/Complete Inspection No Violations Noted Routine site inspection of LPR Energy LLC's Snow Shoe 2 Pad on January 3, 2018. No one was onsite during my inspection. The ESCGP (ESX10-027-0020(01)) covers both the freshwater impoundment (FWI) and the Marcellus well pad. During my inspection I only inspected the well pad and not the FWI. The ESCGP expired on July 10, 2017. In addition, the ESCGP has not been terminated by the Department.

There are 3 producing Marcellus wells located on this well pad. Each well is tagged with the API and well permit number. The well cellars are filled in will gravel. Connected to each well heads is a 275 gallon methanol tote stationed in secondary containment. Each well is connected to its own gas production unit (GPU). Gas from each GPU is metered before discharging into Superior Appalachian's 8" steel pipeline. In addition, there are (3) 400 BBL produced fluids tanks stationed on the well pad in modular secondary containment. Water within this containment was frozen.

There are total of 3 producing wells on this pad. Annulus is closed and not vented at time of inspection. Mechanical gauge on the production line reads pressure 900 PSI, mechanical gauge on the annulus reads pressure ¿ 0 PSI. 0% combustible gas was detected at the annulus or in the area immediately surrounding the well. 2833567 Primary Facility 2019-01-23 Routine/Complete Inspection No Violations Noted Routine site inspection of LPR Energy LLC's Snow Shoe 2 Pad on January 23, 2019. No one was onsite during my inspection. The original ESCGP (ESX10-027-0020) was issued to Enerplus Res (USA Corp on October 21, 2010 with an expiration date of October 20, 2015. A major modification was issued for this permit (ESX10-027-0020(01))to Chevron Appalachia LLC on July 12, 2012 with an expiration date of July 11, 2017. Both the original and amended permits cover both the freshwater impoundment (FWI) and the Marcellus well pad. During my inspection I only inspected the well pad. The FWI was not accessible due to snow and ice covered road. The ESCGP expired on July 10, 2017. This ESCGP has not been terminated to date by the Department.
