Clearfield county conducted October 14, 2016. At the time of inspection; a service rig was rigged up on the well for plugging ops. I talked to Matt from Fairman Corp to discuss operations on the pad. The original plugging procedure was revised to swap the proposed bottom cement plug in the 4 ½” in exchange for (5) mechanical bridge plugs (3220’, 2940’, 2740’ 2420’ 2220’) because there is a hole in the 4 ½” and the operator does not want to lose cement returns to the annulus or possibly cement the 4 ½” in place above where they plan to cut & pull csg. The rig mixed and pumped gel (ratio of appox. 1 sk per 5bbls) to load the hole yesterday as nonpourous material between plugs as per the regulations. When I arrived on location Surefire Wireline was on location setting the last two bridge plugs. Til I left they were running back in the hole to shoot off 4 ½” (2144’). The rig will pull 2144’ of 4
2530526 Primary Facility 2016-10-21 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of Fairman Corporation’s Walter E Dunlap (#2, API Permit # 033-20494) conventional gas well in Sandy Twp. Clearfield county conducted October 21, 2016. At the time of inspection; a service rig was rigged up on the well for plugging ops. I talked to Matt from Fairman Corp to discuss operations on the pad. All the bridge plugs have been set and 4 ½” shot off and pulled above the last bridge plug. Waiting on cement crew to arrive to mix, pump, and spot last balanced plug from the stub back up past the 8 5/8” shoe and into the casing. Universal Cementers showed up. Went over depths and volumes. When the pump operator came online to pump through tubing to establish circulation he pressured right up. The rig crew will now trip out of the hole to investigate blockage at bottom of tubing. I left location at this time, plan to return Monday.
2531681 Primary Facility 2016-10-24 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of Fairman Corporation’s Walter E Dunlap (#2, API Permit # 033-20494) conventional gas well in Sandy Twp. Clearfield County conducted October 14, 2016. I spoke with Matt from Fairman to discuss operations on the pad since I left Friday when they rig was tripping out tubing. They use a perforated piece of tubing at the bottom of the workstring to set plugs and when they pulled the tubing out of the hole the last joint was plugged, reason for them to pressure up when coming online to establish circulation Friday. The rig cleaned out the tubing, tripped in the hole and broke circulation on Friday with about 90 bbls. Universal was scheduled to cement today but got delayed, expected to be on location tomorrow at 8 am to cement last plug from the 4 ½” stub, the openhole, back into the 8 5/8” casing.
2534400 Primary Facility 2016-11-02 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of Fairman Corporation’s Walter E Dunlap (#2, API Permit # 033-20494) conventional gas well in Sandy Twp. Clearfield County conducted November 2, 2016 at 12 pm. The operator has finished plugging and the rig has pulled off location. At the time of inspection; The 8 5/8” casing has monument cap with 3” vent pipe. I isolated the vent w/plastic to my Altair 5x gas meter; no gas detected at this time. The (11 ¾” x 8 5/8”) annulus was exposed, I poured water on the annulus; no bubbling, no gas detected. Waiting on plug cert.
2775142 Primary Facility 2018-09-11 Site Restoration No Violations Noted Restoration inspection on September 11, 2018 with Santo Adams of Fairman Corporation at restored conventional well 033-20494. The well has been plugged and the area surrounding the plugged well has been restored. The restored site is very well vegetated.

Per this inspection I approve the well site restoration report submitted for 033-20494.
