2746280 Primary Facility 2018-06-20 Routine/Complete Inspection Pending The weather is 80 Deg. F and cloudy at time of inspection. Routine inspection of Kriebel Wells Dissolution Co. LLC., Scull Pad Well # 1 on June 20, 2018.

There is 1 producing well on this pad. Production processing equipment includes two separators, in-line drip and production water tank. Production line pressure is 115 PSI, casing pressure gauge reads 150 PSI. Annulus (7¿ x 4-1/2¿) is open and vented. 0% combustible gas was detected at the annulus. Observed gas bubbling outside of the surface (7") casing and inside (13-3/8") conductor. Conductor depth is 44 ft. sanded in, surface casing depth is 1103 ft., cemented in the hole. Discussed further investigation of this issue with company representative, that is present on location. No violations are noted at time of inspection, but needs follow up inspections. Kriebel Well Services, Inc. Representative ¿ Denny Mattis, Field Foreman. DEP Representative ¿ Ulyana Trynovich, Oil and Gas Inspector.
