1824544 Primary Facility 2009-08-13 Drilling/Alteration No Violations Noted Site inspected with OGI Bruce Jankura. Well has been completed and flaring is in progress. Chris Abshire, Well test technician was on site. All disturbed areas have been seeded and mulched with about 45% vegetation already grown back in. Site has brush row reinforced with line of silt fence surrounding parameter. All E&S control BMPs have been installed properly and are working adequately. Flowback fluids are being directed into pit then pumped from pit into tanks. One hose had a small hole and was spraying a fine mist of fluids which were going back into the pit. Mr Abshire was in the process of changing that hose. The pit is constructed and lined properly. Liner appears sound with no visible cuts or punctures. More than 2ft of freeboard is being maintained. Northwest corner of pad appears to have been redisturbed. Area has fresh seed and mulch and a new section of sediment fence has been installed. The area extends beyond the edge of the pad about 200 ft. Vegetation has not yet begun to grow in this area
1824614 Primary Facility 2009-08-27 Complaint Inspection De minimus Violations Noted Site in response to water quality complaint #265864. A nearby spring and parts of Alex Branch located downstream of well site are showing increased levels of TDS and excessive conductivity. Field tests of the small water body located on the northern edge of pad also indicated these raised levels. All fluids have been removed from site. Spoke with Ted Cunkelman of EOG. Flaring was to be complete by evening. Site restoration has begun. Fill side was being dug up and redistributed on cut side. Pit remains open at this time. E&S BMPs are still in place. In area where vehicles park, a small depressed area about 4 Ft in diameter has begun filling with sediments. permit # is not completely filled in on sign. This inspection was conducted jointly with the following individuals from the PA Fish and Boat Commission:
1829040 Primary Facility 2009-09-04 Routine/Partial Inspection Violation(s) Noted Miller - on-site meeting w/EOG & PaFBC reps, multiple investigations between 8/25/09 & 9/4/09 resulted in determination that activities at this well site released contaminants from Marcellus shale drilling to atmosphere, written response requested w/in 10 days. 1829448 Primary Facility 2009-09-04 Follow-up Inspection Violation(s) Noted Inspected during meeting between DEP O&G staff, PA Fish and Boat Commission and Representatives of EOG Resources Inc,. Site has been recontoured, limed, seeded and mulched. last five digits of permit number have been filled in. E&S controls remain in place and working properly. Inspection/meeting revealed that hydralic fracturing flowback fluids had previously been released into air and deposited in area surrounding site. Recent rains may have carried substances into local surface waters. Samples taken on 27Aug2009 show elevated contaminants related to marcellus drilling.
