Routine inspection of Greylock Energy, Coldstream Affiliates Pad, well number 1MH on February 20, 2018. There are two producing wells on this pad. Two annulus vents (9.625 in. x 5.5 in.) are closed and not vented at time of inspection. Annulus vent (13.37 in. x 9.625 in.) is closed and vented into the tank. 100% LEL and 25% combustible gas was detected at the gas separator area during inspection. Followed up with company representative via the phone regarding gas equipment leak. Wellsite looks neat and well organized. No violations noted at the time of inspection. DEP Representative ¿ Ulyana Trynovich, Oil and Gas Inspector.

2701875 Primary Facility 2018-03-06 Routine/Complete Inspection No Violations Noted The weather is 40 Deg. F and sunny at time of inspection.

Follow up inspection of Greylock Energy, Coldstream Affiliates Pad, well number 1MH on March 6, 2018. There are two producing wells on this pad. Two annulus vents (9.625 in. x 5.5 in.) are closed and not vented at time of inspection. Annulus vent (13.37 in. x 9.625 in.) is closed and vented into the tank. The production tree top pressure is 92 psi. 100% LEL and 96% combustible gas was detected at the dump level controller on the separator during inspection. Physically could sense the gas odor approximately from 5 ft. away from the gas leak source. Gas rate intensity is much lower compare to the rate intensity during previous inspection. Followed up with company representative via the phone regarding gas equipment leak. No violations noted at the time of inspection. DEP Representative ¿ Ulyana Trynovich, Oil and Gas Inspector.

2729885 Primary Facility 2018-05-15 Routine/Complete Inspection No Violations Noted The weather is 72 Deg. F and sunny at time of inspection.

Routine inspection of Greylock Energy, Coldstream Affiliates Pad, well number 3MH on May 15, 2018. There are two producing wells on this pad. Two annulus vents (9.625 in. x 5.5 in.) are closed and not vented at time of inspection. Annulus vent (13.37 in. x 9.625 in.) is closed and vented into the tank. The production Xmas tree top pressure gauge reads 64 psi. 0% combustible gas was detected in the area during inspection. Missing bolt at the dump level controller has been replaced, where 100% LEL and 6% combustible gas was detected, but I did not sense any gas odor around gas separator unit.
