2840571 Primary Facility 2019-02-15 Routine/Complete Inspection No Violations Noted Routine site inspection of Punxsy Hunt Club 21V (033-26890) well. Seneca Resources submitted a plugging notification on 2/7/19 to begin plugging the well on 2/8/19. Goodwin and Plants Inc. from Shinglehouse, PA is the plugging contractor. During my inspection the contractor was in the process of plugging the well. As of my inspection they cemented up to 3,000 ft. Workover rig #12 is onsite. A total of 4 frac boxes are onsite. One box is being used for storage, 2 boxes are holding freshwater for cementing, and 1 box holds waste water from cementing. All equipment including the rig is located on bermed secondary containment liner. No E&S issues observed.

No violations.

2840823 Primary Facility 2019-02-15 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection of this vertical, uncompleted, unconventional well was conducted February 15, 2019. At the time of inspection, Plants and Goodwin Rig 12 is on location for final plug and abandonment. WL is rigged up to perforate 4 1/2" casing. I met with the rig supervisor to discuss operations on the pad. Bottomhole cement plug has been pumped. They tagged at 2300'. A CBL has been run on the 4 1/2" (cemented to surface) after discovering pressure on the annulus. Pressure build-up over night was 2 PSI. The CBL indicated ~150' of free pipe formation where wireline are perforating 4 1/2" at this time (1500'-1650'). 4 1/2" was lined out from BOP to open-top container; nothing felt at the vent after perforating and no gas was detected. 8 5/8" was shut-in to gauge; no pressure change after perforating. I discussed contingency plan with the operator if cement squeeze is unsuccessful. No obvious major issues were observed at this time.
2842968 Primary Facility 2019-02-21 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection of this vertical, uncompleted, unconventional well was conducted February 21, 2019. At the time of inspection, Plants and Goodwin Rig 12 is on location for final plug and abandonment. I met with the rig supervisor to discuss operations on the pad since my last visit when WL was on location to perforate for cement squeeze behind 4 1/2" (1500'-1650'). They shut-in both 4 1/2" and 8 5/8" casings over the weekend after perforating. Shut-in pressures were approximately 10 PSI on the 4 1/2" and 5 PSI on the 8 5/8". They spotted cement across perfs on Monday, POOH above plug, shut in BOP, pressured up to 500 PSI and proceeded to squeeze approximately 1.5 bbls of cement. WOC and ran in to tag the next day; cement fell back 1540' and 4 1/2" was on vacuum. Proceeded to squeeze exposed perfs which took 1/2" bbl. WOC and tagged at 1380'. 8 5/8" pressure decreased to 2.7 PSI. No pressure reported on the 4 1/2". Overnight shut-in pressure decreased to 1 PSI on the 8 5/8". At the time of in
2844953 Primary Facility 2019-02-26 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection of this vertical, uncompleted, unconventional well was conducted February 26, 2019. At the time of inspection, Plants and Goodwin are rigging down following plug and abandonment. At the wellhead, both 4 1/2" (unrecoverable) and 8 5/8" casings are open to atmosphere. There was no audible or visible venting or discharge observed. No gas was detected at the opening for 4 1/2" casing with an Altair 5x gas meter. A trace of gas (approx. 5% LEL) was detected at the mouth of the 8 5/8" opening. Significant, progressive decreases in pressure to what is now only a trace of gas on this annulus following the cement squeeze behind 4 1/2" casing seemingly indicate a successful squeeze. The operator intends to monitor the well at this time to verify this.
2846970 Primary Facility 2019-03-01 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (post-plugging) inspection of this vertical, uncompleted, unconventional well was conducted March 1, 2019. At the time of inspection, Plants and Goodwin Rig 12 and crew are off location following plugging operations. At the wellhead, 4 1/2" casing (unrecoverable) is shut in. 8 5/8" casing is open at the wellhead to a digital gauge which may be isolated by needle valve (no display on gauge). The location was snow-covered; no evidence of discharge observed. No gas was detected in the well cellar with an Altair 5x gas meter. The operator is in the process of monitoring the well at this time to verify the adequacy of plugging.
2854140 Primary Facility 2019-03-12 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (post-plugging) inspection of this vertical, uncompleted, unconventional well was conducted March 12, 2019. At the time of inspection, Plants and Goodwin Rig 12 and crew are off location following plugging operations. The operator is in the process of monitoring the well at this time to verify the adequacy of plugging. At the time if inspection, 4 1/2" casing (unrecoverable) is shut in at valve atop casing swage. 8 5/8" casing is open to atmosphere; no visible or audible venting or discharge. No gas was detected at the mouth of the vent opening with an Altair 5x gas meter.
2855735 Primary Facility 2019-03-19 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at Seneca Resources Punxsutawney Hunt Club E16-PHC-F well pad on March 19, 2019. No one was onsite during the inspection. The operator recently had well 033-26890 plugged. A frac tank remains onsite with other various miscellaneous plugging equipment. The Department recommends removing all equipment and miscellaneous materials from this site if they are no longer needed. Large ruts and mud was observed throughout the well pad and several disturbed soil areas were observed on the slopes of the well pad. The Department recommends grading the well pad and seeding and mulching all disturbed areas in accordance with Chapter 102.22. Department staff will be conducting a follow up inspection to see if seeding and mulching issues have been addressed.
