Regulatory Inactive Status for this well has been granted in May 2013, expires - on April 22, 2019. All 3 annulus vents is are open and vented, plumed to the tote. 20% LEL and 0% combustible gas was detected at the plastic tote, 0% combustible gas was in the area immediately surrounding the well. Observed exposed cellar filled with water. Dark in color and heavy in density fluids, that looks like a drilling mud, are discharged via rubber hoses from all 3 well¿s annulus vents into 1 plastic tote on location. Some of the fluids are being spilled in the area immediately surrounding the tote. Plastic tote, labeled as Produced Water, has puncture holes on the top of it, and hoses are inserted into these holes, not secured from falling out of it. The area where drilling fluids has been spilled on the ground was cleaned up. Tank has been placed on the containment. Orange guardian safety barrier has been placed around these wells. No violations are noted at time of inspection.

A Notice of Intent to Plug the SGL 90 19H has been submitted by the operator. At the time of inspection, gate to access road was locked and the plugging contractor was waiting at the gate. I reviewed and discussed the tentative plugging procedure with the plugging contractor. I walked up the access road to location. There were three inactive wells on the pad; the 18H, 19H, and 20H. All wells had a night cap bolted on casinghead flange. All well cellars were full of water with three annular vents extending above ground level, tied together, open, and lined out to a container through a rubber hose. No bubbling was observed in the cellars. A trace of gas was detected in the container. No obvious major issues were observed. |

On November 21, 2018 Department representative Sue Herzing conducted a routine site inspection at the above listed well pad. The plugged rig was onsite. The plugging contractor "Plants & Goodwin" were onsite working on the plugging operation. All plugging related equipment was being stationed on secondary containment. No signs of liquid spills outside of the secondary containment. No violations noted. |

They ran in and tagged plug with sandline at the time of inspection; tagged at 492'. No issues were observed or reported. They will top off with gravel to surface and move to the next well, the 20H, for plug and abandonment. |
