2772345 Primary Facility 2018-09-07 Complaint Inspection Violation(s) Noted Department representatives Sue Herzing and Ulyana Trynovich conducted a complaint inspection with Colleen Shannon of the PA Game Commission at Seneca Resources D-17 SGL 90 C well Pad on September 7, 2018.

We observed 3 exposed well cellars (photo 1). Water was observed in each well cellar. A conductivity and pH reading were taken from well cellar 033-26976: 256 uS/cm and 7.06 pH units respectively. A sheen was observed within this well cellar (photo 2). We observed black hoses attached to each separate well head flowing fluids black in color to a nearby tote labeled produced water and natural gas (photo 3). Puncture holes were observed at the top of the tote (photo 4), allowing the hoses to drip in fluids within the tote. The tote is not located on secondary containment. The fluids level within the tote were approximately 1/2 full. Black fluids and/or staining were observed underneath the tote and surrounding the tote in an approximate 6'x6' area (photos 5-7). 2777145 Primary Facility 2018-09-07 Routine/Complete Inspection No Violations Noted The weather is 60 Deg. F and cloudy at time of inspection. Routine inspection of Seneca Resources Corp., SGL Pad, Well # 20H-90 on September 7, 2018. There are 3 inactive and shut in wells on this pad. Regulatory Inactive Status for this well has been granted in May 2013, expires - on April 22, 2019. All 3 annulus vents is are open and vented, plumed to the tote. 20% LEL and 0% combustible gas was detected at the plastic tote, 0% combustible gas was in the area immediately surrounding the well. Observed exposed cellar filled with water. Dark in color and heavy in density fluids, that looks like a drilling mud, are discharged via rubber hoses from all well¿s annulus vents into 1 plastic tote on location. Some of the fluids are being spilled in the area immediately surrounding the tote. Plastic tote, labeled as Produced Water, has puncture holes on the top of it, and hoses are inserted into these holes, not secured from falling out of it. 2778472 Primary Facility 2018-09-21 Follow-up Inspection No Violations Noted The weather is 75 Deg. F and sunny at time of inspection. Follow up inspection of Seneca Resources Corp., SGL Pad, Well # 20H-90 on September 21, 2018. There are 3 inactive and shut in wells on this pad. All 3 annulus vents is are open and vented, plumed to tank. 0% combustible gas was detected at the tank, 0% combustible gas was detected in the area immediately surrounding the well. The area, where drilling fluids have been spilled on the ground, was cleaned up. Tank has been placed on the containment. Orange guardian safety barrier has been placed around these wells. No violations are noted at time of inspection.

2804108 Primary Facility 2018-11-13 Routine/Complete Inspection No Violations Noted A routine (pre-plugging) inspection of an inactive, uncompleted well was conducted November 13, 2018. A Notice of Intent to Plug the SGL 90 19H has been submitted by the operator. At the time of inspection, gate to access road was locked and the plugging contractor was waiting at the gate. I reviewed and discussed the tentative plugging procedure with the plugging contractor. I walked up the access road to location. There were three inactive wells on the pad; the 18H, 19H, and 20H. All wells had a night cap bolted on casinghead flange. All well cellars were full of water with three annular vents extending above ground level, tied together, open, and lined out to a container through a rubber hose. No bubbling was observed in the cellars. A trace of gas was detected in the container. No obvious major issues were observed.
2808942 Primary Facility 2018-11-21 Plugging(Includes Plugged/Mined Through) No Violations Noted On November 12, 2018 the Department received a plugging notice to plug Marcellus well 033-26976 located on Seneca Resources D17-SGL 90 C well pad.

On November 21, 2018 Department representative Sue Herzing conducted a routine site inspection at the above listed well pad. The plugged rig was onsite. The plugging contractor "Plants & Goodwin" were onsite working on the plugging operation. All plugging related equipment was being stationed on secondary containment. No signs of liquid spills outside of the secondary containment.

2815143 Primary Facility 2018-12-12 Plugging(Includes Plugged/Mined Through) No Violations Noted The rig is off location following final plug and abandonment of the 19H and 20H. Both are vertical, uncompleted, (now plugged) unconventional wells.

At the time of inspection, both plugged wells are cut below grade, marked, and buried. There were no open holes, discharges, bubbling, or venting observed. No gas was detected with an Altair 5x gas meter at ground level above the location of the buried wells. No obvious issues were observed at this time.
