On 04/27/16 at approximately 1430 hours I conducted a routine complete inspection of the Hamborsky Unit. The Act 9 sign was in place and all required documents were present in the mail box. The site was well vegetated. There were no violations observed at the time of inspection.

The site was well vegetated and there were no signs of erosion at the time of inspection. A portion of the well site had been farmed for corn. The equipment appeared to be in condition. There were no violations observed at the time of inspection.
2685605 Primary Facility 2018-01-22 Routine/Partial Inspection Violation(s) Noted Routine partial inspection due to well site being surrounded by security fence. Well appears equipped for production. Two brine tanks on site. The NOT for this site was approved in 2013.

Department observed erosion gully beginning to form and a small amount of what appears to be water leaving well pad area on NW side of well pad. Department contacted operator rep Chad Eiserman by phone from site. Mr. Eiserman stated that Chevron personnel will be sent to site and he would follow up with department. UPDATE 14:57: Operator rep Curtis Small contacted Department by phone. Mr. Small stated a Chevron rep will be on site today and inspect the NW area of the well pad. Mr. Small also stated that he personally would be on site first thing tomorrow morning and will follow up with Department by phone or email. Department requested operator determine the source of the water leaving the site. Mr. Small stated that information would be provided in the follow up call/email. At time of inspection fluid is leaving site and entering an adjacent field (photo #1). As a result of the fluid leaving the site, an erosion gully has formed and this gully continues into the adjacent field (photo #2). It does not appear that the operator has taken any corrective measures to minimize the run-off in this area since the 1-22-18 inspection. Department recommends operator evaluate and implement measures to address run-off and gully formation. Violation issued on 1-22-18 remains outstanding. A follow up inspection will be conducted at a later date.

2714408 Primary Facility 2018-04-04 Follow-up Inspection Outstanding Violations - Viols Req'd Routine partial follow up inspection to inspection record #2685605 where a violation was issued for failure to comply with E and S requirements.

At time of inspection fluid continues to flow from well pad area on NW side of well pad as observed in previous inspections. This fluid has created a gully that starts at the well pad (photo #1) and continues into an adjacent ag field (photo #2). It does not appear operator has taken any corrective measures to address run-off in this area. Department observed a wet area on the south-facing slope of the well pad (photo #3). This area is outside the well pad fence and is vegetated. Department recommends operator continue to monitor this area to avoid any E and S issues. Violation issued on 1-22-18 remains outstanding. A follow up inspection will be conducted at a later date.

2718219 Primary Facility 2018-04-09 Follow-up Inspection Outstanding Violations - Viols Req'd Routine partial follow up inspection to inspection record #2685605 where a violation was issued for failure to comply with E and S requirements.

At time of inspection, gully observed in inspection record #2685605 remains, and appears to have worsened (photo #1 and 2). No fluid is flowing at this time. It does not appear operator has taken any corrective measures to address run-off in this area. Wet area observed in inspection record #2714408 on the south-facing slope of the well pad remains (photo #3). Department recommends operator continue to monitor this area to avoid any E and S issues. Upon review of operator's Post-Construction Stormwater Management/Restoration Narrative, it appears stormwater runoff is contained by subsurface infiltration beds. According to this narrative, these infiltration beds are to be inspected at least 4 times a year. Department requests a copy of these inspections for the last 2 years. Violation issued on 1-22-18 remains outstanding. A follow up inspection will be conducted

2723185 Primary Facility 2018-04-23 Administrative/File Review Outstanding Violations - Viols Req'd ADMIN inspection to inspection record #2685605 where a violation was issued for failure to comply with 25 PA Code 102 (E and S requirements).

Department reviewed file for ESX09-051-0044 Hamborsky 12H, 13H. According to this file, this ESCGP-1 was issued in 2009 and a major modification was approved on 12-24-2011. The NOT for this permit was approved on 1-22-2013 with the following comments: "Operator was reminded to check vegetative growth in area of reclaimed sed trap in spring 2014 to ensure it continues to have 70% uniform coverage. " Submitted with the ESCGP-1 is a Post-Construction Stormwater Management/Restoration Narrative. This narrative states that water quality is provided through infiltration system, swales, diversion ditches, and seeding and landscaping. This narrative also states that inspections will be conducted on these BMP's on a yearly basis (at least). In inspection record #2718219 Department requested copies of the inspections of the infiltration bed for the last 2 years. As of today, th

At time of inspection wet area on south-facing slope of well pad observed in inspection record #2714408 and inspection record #2718219 appears to have dried and at this time is well vegetated. A level spreader has been installed at the NW side of the well pad (photo #1) where fluid was observed leaving the well pad area (see inspection records #2685605, #2712867 and #2714408). A small amount of fluid is flowing through the level spreader and leaving the site at this time (photo #2). This fluid is entering an area that is currently a vegetated ag field (corn). This fluid does not appear to be creating an erosion gully at this time. Department recommends operator continue to monitor this area to ensure erosion gullies do not develop when ag field is un-vegetated/sparsely vegetated.
