On 11/30/16 at approximately 1300 hours I completed a routine complete inspection of the Nemacolin 7008 well pad in response to an extension request submitted by T & F Exploration. The site restoration had been delayed after T & F halted construction of PCSM when they encountered an unknown material below the pad liner. An investigation took place to determine if the material was possibly drill cuttings disposed of by a previous operator. After investigation by T & F and the Department, it was determined that T & F could continue, but if they encountered any evidence of buried material they should stop and contact the Department. Due to the delay, weather conditions have not been appropriate for vegetative growth. An extension of one year from the date of November the 28th has been approved. The new date for site restoration is November 28th, 2017.

The Act 9 sign was in place and all required documents were present at the site. The site had recently been restored and had temporary stabilization (see photographs). There were no violations observed at the time of inspection.

2778485 Primary Facility 2018-09-19 Routine/Complete Inspection In Compliance with Policy According to Department records, well is in Regulatory Inactive status.

At time of inspection well pad surrounded by security fence and padlocked. Department observes a bbl on the northern corner of well pad. This bbl appears unlabeled and is not on secondary containment. Department also observes fluid in brine tank secondary containment unit. In addition, a hole is cut in the security fence on the SW side of the well pad. Department contacted operator rep by phone from site. Operator rep states he believes the bbl on the northern corner of the site is empty. Department recommends removing items not necessary for the production of the well from the site. Operator rep states that the wells are not in production and the fluid in the secondary containment unit is rainwater. Department recommends operator monitor fluid in the secondary containment unit and dispose of according to current regulations.
