Erosion was occurring along the fence between the pad and former tank farm. There was what appeared to be acid rock drainage in the area of the removed sediment trap. This area also contained a large amount of surface water. The drainage ditch the left side of the access road near the Act 9 sign also contained what appeared to be acid rock drainage. The site had four non haz waste barrels that were un labeled on the pad and two on their sides near the gate to the pad.
2381436 Primary Facility 2015-06-09 Routine/Complete Inspection No Violations Noted On 06/09/15 at approximately 0930 hours I conducted a routine complete inspection of the Polk Unit. All required documents were present at the site. The site was stable and the wells appeared to be in good condition. The location of the former tank farm exhibited no locations of standing water. The erosion rill noted on 04/16/15 had been repaired. The area in the location of the former sediment trap was still wet. Iron producing bacteria were still present at the site. There were no violations observed at the time of inspection

The site was well vegetated and there were no signs of erosion observed. The wellheads appeared to be in good condition. In the location of the restored tank pad a gravel excavated area was observed. I contacted LPR and was informed that the gravel was buried during Chevron Appalachia LLC's reclamation of the site. LPR was excavating the gravel for use on the site in order to construct a tank pad under an E&S plan, since the site has received an NOT. I recommended that LPR contact DEP engineering staff to confirm that an E&S plan was sufficient and no permitting was required.

NOTE: The access gate to this well site is unlocked and open, there is no one on site and the two lifts in the attached photos have the keys left in the ignitions. They are fully operational. It is my recommendation that the operator not leave keys in the ignitions of unattended equipment as I have seen well sites and equipment damaged and vandalized by equipment which had the ignition keys left in them when unattended.
