2504294 Primary Facility 2016-05-24 Routine/Complete Inspection No Violations Noted

On 05/24/16 at approximately 1400 hours I conducted a routine complete inspection of the Frankhouser Unit. The Act 9 sign was in place and all required documents were present in the mailbox. The site was well vegetated with the exception of one location at the far rear of the well site that has previously been addressed by Chevron. The location had been seeded and mulched by Chevron, but the location still exhibited sparse vegetation. It is recommended that the location be addressed if vegetative growth does not occur. There were no violations observed at the time of inspection.

2586233 Primary Facility 2017-04-24 Routine/Partial Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The production equipment appeared to be in good condition. Several small areas of sparse vegetation had received additional topsoil, seed, and mulch. The site overall was well vegetated and there were no signs of erosion observed. Once the areas of sparse vegetation reach uniform 70% vegetative cover the site will meet the requirements for NOT approval. There were no violations observed at the time of inspection.
