2597218 Primary Facility 2017-05-19 Routine/Complete Inspection No Violations Noted On 05/18 and 05/19 Chevron had Civil and Environmental Consultants (CEC) perform soil testing and vegetative analysis in response to a Complainant alleging soil compaction had occurred on the site. The Complainant had hired a consultant to conduct an investigation to look for evidence of compaction. The Department and Chevron are in possession of the report completed by the consultant post investigation.

The Complainant

2626508 Primary Facility 2017-08-03 Routine/Partial Inspection No Violations Noted On 06/15/17 Chevron Appalachia LLC submitted a request for extension of the ESCGP. As of 07/31/17 the extension request had not been approved and the permit expired.

On 08/03/17 I conducted a routine partial inspection of the site. The Act 9 sign was in place and all required documents were present at the site. The portion of the site below the well pad appeared to have been recently mowed (see photographs). The portion above the well pad had not been mowed. The site was well vegetated and there were no signs of erosion found at the time of inspection.

An unfilled test pit from the Rettew soil investigation was observed below the well pad (see photographs). The pit contained water at the time of inspection.

A wet location was observed on the lower portion of the site (see photographs). The wet area appeared to be located in a swale. The area appeared to exhibit wetland vegetation. A shallow depression with standing water was observed in this location. The depression appeared to be caused by equipment us

2760069 Primary Facility 2018-08-01 Routine/Partial Inspection No Violations Noted On 08/01/18 at approximately 1345 hours I conducted a routine partial inspection of the site. The Act 9 sign was in place and all required documents were present at the site. Chevron had recently installed a french drain type system below the well pad to alleviate a wet spot in the field. Accelerated erosion and poor vegetative cover were observed in the recently disturbed area (see photographs). It is recommended that the erosion be repaired and additional seed and mulch be applied. Several small pieces of scrap black pipe were observed. It is recommended the scrap material be removed.
