On 03/04/16 at approximately 0830 hours I conducted a routine complete inspection of the Baily Unit. The Act 9 sign was in place and all required documents were present in the mailbox.

The site was stable. No E&S issues were observed at the time of inspection, but light snow cover was present. 3 ASTs were present at the time of inspection. The containment around the tanks was in good condition, but one corner of the containment was falling in (see photograph). It is recommended that this containment be addressed.

On 06/07/16 at approximately 1200 hours I met with Curtis Small of Chevron Appalachia LLC to discuss a stockpile at the Baily Unit. The Act 9 sign was in place and all required documents were present at the mail box.

The stockpile in question was placed on the AST pad and consisted of soil and stone from the well pad that was left over from the installation of the GPUs and connecting them to their respective well heads.

Due to the location of storage and the stability of the stockpile approval was given for it to remain in place until it is used during the reclamation of the site. There were no violations observed at the time of inspection.
2581720 Primary Facility 2017-04-12 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The site overall was well vegetated, but several small bare spots were observed at the site. It is recommended that these areas be seeded and mulched. The production equipment appeared to be in good condition. There were no violations observed at the time of inspection.

The well site was well vegetated with the exception of the portion of the site where the AST pad was located. The AST location had been recently restored, seeded, and mulched. Some vegetation was present in the restored area. There were no signs of erosion observed. There were no violations observed at the time of inspection.

2768758 Primary Facility 2018-08-23 Routine/Partial Inspection No Violations Noted On 08/23/18 at approximately 11 hours I met Curtis Small of Chevron to discuss third party impacts at the site. Chevron indicated their intent to file for an NOT and requested an inspection to document third party impacts. A third party had sprayed herbicide on banks, fence lines, and ditches on the site to control the vegetation. Chevron indicated photographs and supporting evidence of the third party impacts would be submitted to the Department at the time of the NOT submission. Chevron also indicated a land owner consent form allowing a sediment trap to remain on the site for land owner use would be submitted at the time of inspection. There were no violations observed at the time of inspection.
2783573 Primary Facility 2018-10-03 Site Restoration No Violations Noted A site restoration inspection was conducted in response to an NOT submittal from Chevron. The Act 9 sign was in place and all required documents were present at the site.

The production equipment appeared to be in good condition. The site was well vegetated and there were no signs of erosion observed. Site vegetation met and exceeded 70% uniform vegetative cover. PCSM features were installed and appeared to be functioning. The site meets the requirements for permit termination. The NOT is approved.

2838028 Primary Facility 2019-02-08 Routine/Complete Inspection No Violations Noted This well is tagged and in production. The well head casing, piping, production set equipment, containment dikes and tank battery all look good.

The uniform well and equipment placement, neatness, mechanical integrity appearance and housekeeping of the wells and support equipment on this large multi-well pad is excellent and among the best that I have seen anywhere in the Commonwealth of Pennsylvania.
