2694564 Primary Facility 2018-02-14 Routine/Complete Inspection No Violations Noted On Wednesday, February 14, 2018, I conducted a routine-complete inspection at the Charles Nelson No. 1 well site. The Nelson No. 1 well is a vertical gas well located in Center Township, Greene County.

The access road and the pad area were in good condition and the site has been stabilized with vegetative growth.

The earthen berm secondary containment structure constructed around the production tank was observed to be vegetated and in tact. There were no apparent signs of any fluid releasees.

The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The well was labelled.

No violations were identified during the inspection.

2834048 Primary Facility 2019-01-22 Routine/Complete Inspection No Violations Noted On Tuesday January 22, 2019 I conducted a routine-complete inspection of the Nelson 1 Vertical Gas Well located in Center Township, Greene County.

The access road and the pad area were in good condition.

Uniform vegetative growth was observed through a thin to sparse coating of snow on the ground.

The earthen berm secondary containment around the production tank appeared to be in good condition and observed to be vegetated through the snow cover. The production tank also,appeared to be in sound condition.

Although the operator is compliant with current regulations, they should consider utilizing secondary containment constructed of impermeable materials. A release of fluid within the berm can result on a violation being issued to the operator.

The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The well was tagged with identification.

No violations were identified during the inspection.
