2834081 Primary Facility 2019-01-22 Routine/Complete Inspection No Violations Noted On Tuesday January 22, 2019 I conducted a routine-complete inspection at the Nelson 2 Vertical Gas Well Site located in Center Township, Greene County.

Access to the gas well is shared with the Nelson 3H Oil Well which is located on a separate elevated pad. The access road and the pad area for the Nelson 2 Gas Well was observed to be in good condition.

The site has been stabilized with vegetation which was observed through a thin to sparse cover of snow.

There is no secondary containment around the production tank which is compliant with current regulations given the size of the tank. The production tank appeared to be in sound condition.

Although the operator is compliant with current regulations, they should consider utilizing secondary containment constructed of impermeable materials. A release of fluid from the tank can result in a violation being issued to the operator.

The gas well and associated infrastructure appeared to be in good condition and functioning properly. The well was tagged with
