2694581 Primary Facility 2018-02-14 Routine/Complete Inspection No Violations Noted On Wednesday, February 14, 2018, I conducted a routine-complete inspection at the Relosky No. 1 Well Site. Unit 1 is a vertical gas well located in Center Township, Greene County.

The access road and the pad area were in good condition and the area has been stabilized with vegetation.

There is no secondary containment structure around the production tank. The tank appeared to be in good condition and uncompromised. There were no apparent signs of fluid release.

The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The well was labelled.

No violations were identified during the inspection.

2849160 Primary Facility 2019-03-05 Routine/Complete Inspection No Violations Noted On Tuesday March 5, 2019 I conducted a routine-complete inspection at the Relosky Number 1 Conventional Vertical Gas Well Site located in Center Township, Greene County.

The access road and the pad area were in good condition.

Uniform vegetative growth was observed through a thin cover of snow on the ground.

The production tank appeared to be in good condition. There is no secondary containment used in association with the production tank. Although the operator is in compliance with current regulations, they should consider utilizing secondary containment constructed of impermeable materials. A release of fluid from the tank can result in a violation being issued to the operator.

The gas well and associated infrastructure appeared to be in good condition and functioning properly. The well was tagged with identification.

No violations were identified during the inspection.
