2610413	Primary Facility	2017-06-16	Compliance Evaluation	Violation(s) Noted	On Friday, June 16, 2017, I conducted a compliance evaluation inspection at the burper valve assembly (SL-GH56-01A) associated with the GH56 CBM well (059-24403) located in Center Township, Greene County. The inspection was conducted to evaluated remedial activities conducted in response to a production fluid release from the valve assembly which was reported to the Department on or about April 25, 2017. As a consequence of the release, violations of 25 Pa. Code Section 78.54 and 35 P.S. 6018.301 were cited and sent to the operator on May 2, 2017 (inspection no. 2589040, violation nos. 784592 and 784593, enforcement no. 352985). It had been determined, during an April 27 inspection while remedial activities were being conducted, that the estimated quantity of released fluid (10 gallons) provided by the operator had been grossly underestimated. 2627302	Primary Facility	2017-08-02	Compliance Evaluation	No Violations Noted	On Wednesday, August 2, 2017, I conducted a Compliance Evaluation Inspection (CEI) at the burper valve assembly (SL-GH56-01A) associated with the GH 56 CBM well (059-24403) located in Center Township, Greene County. The inspection was conducted in response to a production fluid release in April 2017 which was reportedly remediated and inspected again on June 16, 2017. The June 2017 inspection identified either an incomplete remediation and/or an existing source. Consol Energy-CNX Gas (CNX) returned to the site on Saturday July 10, 2017 to remediate the impacted area. A soil conductivity probe was used to guide the excavation and delineate the impacted area. All excavated materials were disposed off-site. The excavated area was seeded and mulched following earthwork. Signs of heavy cattle traffic were apparent during the inspection. The access road from Patterson Creek Road is also utilized to access the Hildreth GH54 CBM well which is located at higher elevation just beyond the GH56 CBM well. The access road and the pad area were observed to be in good condition. The area has been stabilized with vegetation. The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The well was labelled. There is no on-site production fluid storage. No violations were identified during the inspection.

The access road, which is partially shared with the GH54 CBM Well, and the pad area were observed to be in good condition. The site has been stabilized with uniform perennial vegetative growth. There is no on-site production fluid storage. The gas well and associated infrastructure appeared to be in good condition and functioning properly. The well was tagged with identification. No violations were identified during the inspection.
