2625983 Primary Facility 2017-08-03 Incident- Response to Accident or Event In Compliance with Policy On Thursday, August 3, 2017, I received notification of a release that was discovered the previous afternoon at a burper valve assembly associated with the GH 51 CBM well (059-24405) located in Morris Township, Greene County. I conducted a site visit Thursday morning to assess the situation and gain information relating to the release. Mr. Jessie Valentine from Consol-CNX Gas (CNX) was also present during the site visit along with the well tender and the remediation contractor.

The well tender was on-site Monday, July 31, and everything was in order and the well pressure was good. On Wednesday, August 2, the well tender was back on-site and noticed that the well pressure was down approximately 30 psi. CNX began checking the lines and burper valves associated with the well. The release at the burper valve assembly was discovered at this time. It was estimated that approximately 25 gallons of brine was released from the valve assembly, possibly due to valve fowling. 2656053 Primary Facility 2017-11-01 Incident- Response to Accident or Event Violation(s) Noted On Monday, October 30, 2017, Consol Energy-CNX Gas reported a release of brine water from a burper valve associated with the Hildreth GH51 CBM Well located in Morris Township, Greene County. On October 31, 2017, I spoke on the telephone with Mr. Spencer Johnson from Consol Energy regarding details of the release and the planned remediation. Later the same day, I conducted a site visit to assess the situation.

The burper valve associated with GH51 where the release occurred is located at 39.93827797, -80.29184598, which is in a meadow just northeast of the EQT Production Company (EQT) Phillips Well Pad. The operator was on-site performing work to remove the burper valve assembly and cap the valve connection of the water line. There was still water and some pressure in the line when it was cut to perform the work. Upon cutting the line, the excess fluid in the line was expelled under pressure and released to the ground surface. It was estimated that approximately five (5) barrels of brine water were re

2657977 Primary Facility 2017-11-07 Follow-up Inspection No Violations Noted On Tuesday, November 7, 2017, I conducted a follow-up inspection of the former burper valve location associated with the GH51 CBM well where a release of brine water occurred on October 30, 2017. The location of the former valve and the release is just northeast of the EQT Phillips Pad located in Morris Township, Greene County.

The Phillips Pad was used to access the release and remediate impacted materials. The excavation has been backfilled and a small pile clean fill remains beside the excavation. The excavation and dirt pile is surrounded by orange safety fence. The impacted areas (excavation and travelled path from well pad) have not been reseeded and mulched. There is still some clean fill stockpiled alongside the well pad. A small sign, constructed with a cardboard box and marker, was identified near the path to the excavation which read "No Work Until Thursday or Friday". The sign was signed by the landowner (assumed) with a telephone number.

2661539 Primary Facility 2017-11-17 Compliance Evaluation No Violations Noted On Thursday, November 16, 2017, I conducted a compliance evaluation inspection of the former burper valve location (39.93827797, -80.29184598) associated with the GH51 CBM well located just off the EQT Phillips Gas Well Pad located in Morris Township, Greene County. The inspection was conducted to assess the remediation of a brine release from the former valve which occurred on October 30, 2017 and resulted in the citation of 25 Pa. Code, Section 78.54, of which details are summarized in inspection 2656053.

Inspection of the area confirmed that the excavation has been backfilled and the orange safety fence has been removed. The stockpiled clean fill has been removed and all disturbed areas have been seeded and mulched.

No violations were identified during the inspection.

2711851 Primary Facility 2018-03-27 Incident- Response to Accident or Event Violation(s) Noted On Tuesday, March 27, 2018, I received notification of a produced fluid release that was reported to the Department on the previous day. A release of produced fluid occurred in a buried waterline associated with the GH51 CBM well located in Morris Township, Greene County. On Tuesday, I spoke on the telephone with Mr. Mike Farrah from CNX Resources Corporation (CNX) concerning details of the release and the remediation plans. Later that morning, I conducted a site visit to further assess the situation.

The release occurred at a former burper valve location associated with the GH51 CBM well. The release was discovered on Monday during a routine inspection of the waterline RoW. It was estimated that approximately five (5) barrels of produced fluid had been released from the waterline. The release resulted from a leak that had developed in the waterline at the point where the burper valve had been removed and the line was coupled (approximately 39.938277, -80.291845). 2729730 Primary Facility 2018-05-11 Compliance Evaluation No Violations Noted On Thursday, November 16, 2017, I conducted a compliance evaluation inspection of the former burper valve location (39.93827797, -80.29184598) associated with the GH51 CBM well located just off the EQT Phillips Gas Well Pad located in Morris Township, Greene County. The inspection was conducted to assess the remediation of a brine release from the former valve which occurred on March 27, 2018 and resulted in the citation of 25 Pa. Code, Section 78.54, of which details are summarized in inspection 2711851.

The impacted area has been remediated and post excavation soil samples were collected, in accordance with Act 2 guidelines, to confirm that all impacted materials have been removed. Upon receipt CNX Resources will submit the the results to the Department for review. The excavation has been backfilled and all disturbed surfaces have been restored and stabilized with seed and mulch.

No violations were identified during the inspection.
