2779996 Primary Facility 2018-09-20 Complaint Inspection Violation(s) Noted On 09/20/18 the Department received a complaint alleging a slip had occurred in the early part of 2018 at the Yost #6 well.

On 09/20/18 at approximately 1140 hours I conducted an inspection of the site and observed an approximately 350'x35' slip. The slip exposed pipe attached to the GPU and was within 15' of the brine tank. The slip extended up slope of the well. Due to safety concerns, the well could not be closely inspected for integrity. The ground exhibited additional cracking upslope of the well.

Environmental Group Manager Dave McDermott contacted Kirk Elkin of Diversified and requested DGO staff assess the site the following week and provide a plan to remediate the slip.

2793022 Primary Facility 2018-10-12 Follow-up Inspection In Compliance with Schedule On 10/12/18 at approximately 1245 hours I met Kirk Elkin of Diversified Gas & Oil at the site to discuss slip remediation. DeFrank construction representatives and Geotechnical Engineer Scott Bush were also present.

The site remains unstable and the violations are outstanding. I informed Mr. Elkin he would need to consult with OGI supervisor Scott Sabocheck to discuss well integrity, submit a landslide checklist to Department engineering staff, discuss proposed remediation methods with Department engineering staff, and provide an updated E&S plan for the remediation.

The Department requests three copies of Geotechnical Engineer Scott Bush's recommendations be provided with the NOV response.
