Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1923817 Primary Facility 2010-08-31 Site Restoration No Violations Noted Restoration extension inspection. Approved for additional 6 months.
1974801 Primary Facility 2011-03-29 Routine/Complete Inspection No Violations Noted The Department was at the above referenced Marcellus Well to conduct a Routine Inspection. The Well is in production. The site is in over-all good condition. The site has been restored had has obtained 70% uniform vegetative cover. The silt fence needs removed on this site. There is still 1 sediment trap on site. The Departments recommends removing the sediment trap because it is in the natural drainage area is will block natural flow of water during a rain event.
2164738 Primary Facility 2013-04-17 Routine/Complete Inspection No Violations Noted The Department was at the above referenced Marcellus Well site to conduct an inspection of the impoundment construction activities. The Department met on site with both Atlas and Chevron to discuss the encapsulated pit that was on site. The cuttings pit was from an Atlas vertical Marcellus Well. This Vertical Marcellus Well is in production. Chevron is constructing an impoundment for the Johnson wells. During construction activities, Chevron was stockpiling soil on top of the encapsulated impoundment and pushed the top cover off of the cuttings pit. Chevron agreed to work with Atlas and the Department to correct the situation. The cuttings will be excavated and properly disposed at the landfill is the current plan. The Department reminded both Companies that an OG-71 may be needed for alternate waste methods. This inspection report will be sent to both Operators and will be entered into the system for the Headlee Well site and the Johnson Well site.
2297036 Primary Facility 2014-07-08 Routine/Complete Inspection No Violations Noted On 7/8/2014, I performed an inspection of Atlas Headlee #11 Well site. The site is located in Greene Twp., Greene County. The inspection was a routine inspection.
