Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2112732 Primary Facility 2012-10-31 Drilling/Alteration No Violations Noted CROWN RIG 5 DRILLING FOR CHEVRON. WERE DRILLING WATER PROTECTIVE STRING WERE AT 50 ' AT TIME OF INSPECTION. SITE LOOKED GOOD AT TIME OF INSPECTION.
2125877 Primary Facility 2012-12-21 Drilling/Alteration No Violations Noted TOP HOLE DRILLING COMPLETE. SITE COMPLIANT AT TIME OF INSPECTION.
2195700 Primary Facility 2013-08-07 Drilling/Alteration No Violations Noted Top hole drilling complete. Patterson Rig 339 is on site and will be skidding to well and drilling lateral when they complete drilling of well they are currently on.
2285001 Primary Facility 2014-06-11 Routine/Complete Inspection No Violations Noted The access road was in good shape, except for the top section near the well pad. This area was eroded and the operator will need to address the issue.
2294376 Primary Facility 2014-07-30 Routine/Complete Inspection Violation(s) Noted CHEVRON HAD RECENTLY FRACKED THIS WELL AND HAS GAS COMING TO SURFACE BETWEEN THERE 8-5/8 AND 11-3/4 CASING SETS. CHEVRONS CONTRACTOR WAS ON SITE TO INSTALL A PACK OFF ON THE 8-5/8 CASING AT SURFACE. THEY WILL THEN PLUMB THIS GAS INTO THERE PRODUTION TANK AND INSTALL A VALVE AND GAUGE IN LINE FOR MONITORING SURFACE GAS PRESSURES. I SCHEDULED A FOLLOW UP VISIT TO CONFIRM PACK OFF IS FUNCTIONAL.
2294430 Primary Facility 2014-08-05 Compliance Evaluation No Violations Noted FOLLOW UP VISIT FROM 7/30/2014 IN REGARDS TO SURFACE GAS FROM WELL. PACK OFF IN PLACE. MONITORING GAUGE HAD A READING OF 75 PSI. SURFACE GAS PLUMBED INTO PRODUCTION TANK AND APPEARED TO BE FUNCTIONAL BY OPENING RELIEF VALVE.
2309449 Primary Facility 2014-07-31 Routine/Complete Inspection No Violations Noted The well casing near the surface of the ground continues to leak gas, but the operator is on site to mitigate with a casing plug.
2318454 Primary Facility 2014-09-04 Routine/Complete Inspection No Violations Noted Well being flowed back for production.
2564878 Primary Facility 2017-02-24 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and required documentation was present. After a phone conversation with Chevron it was determined that the Department had issued a major modification approval in October 2016. It is recommended that a new E&S plan and major modification approval letter be place in the mail box. The equipment appeared to be in good condition. The site was well vegetated and Chevron indicated restoration of the site is planned for 2017. There were no signs of erosion observed at the time of inspection nor were violations observed.
