Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2112735 Primary Facility 2012-10-31 Drilling/Alteration No Violations Noted CONDUCTOR SET WAITING OR TOP HOLE RIG TO MOVE AND DRILL. SITE LOOKED GOOD AT TIME OF INSPECTION.
2125878 Primary Facility 2012-12-21 Drilling/Alteration No Violations Noted TOP HOLE DRILLING COMPLETE. SITE COMPLIANT AT TIME OF INSPECTION.
2195698 Primary Facility 2013-08-07 Drilling/Alteration No Violations Noted Well drilled to completion and ready for stimulation process.
2285010 Primary Facility 2014-06-11 Routine/Complete Inspection No Violations Noted Well in compliance.
2294334 Primary Facility 2014-07-30 Routine/Complete Inspection Violation(s) Noted CHEVRON HAD RECENTLY FRACKED THIS WELL AND HAD GAS COMING TO SURFACE BETWEEN THERE 8-5/8 AND 11-3/4 CASING SETS. CHEVRON HAS INSTALLED A PACK OFF ON THE LEAKING CASING AT SURFACE. THERE IS A 1" LINE FROM THE PACK OFF TO RELIEVE CASING PRESSURE THROUGH THERE PRODUCTION TANKS. IN THERE 1" LINE THEY HAVE A GAUGE AND VALVE INSTALLED TO MONITOR SURFACE GAS PRESSURE. THE GAUGE WAS READING 80 PSI ON IT AT TIME OF INSPECTION. CELLAR WAS FILLED WITH GRAVEL COULD NOT VISUALLY INSPECT PACK OFF.
2294429 Primary Facility 2014-08-05 Compliance Evaluation No Violations Noted FOLLOW UP VISIT FROM 7/30/2014 IN REGARDS TO SURFACE GAS FROM WELL. PACK OFF IN PLACE. MONITORING GAUGE HAD A READING OF 140 PSI. SURFACE GAS PLUMBED INTO PRODUCTION TANK AND APPEARED TO BE FUNCTIONAL BY OPENING RELIEF VALVE.
2309451 Primary Facility 2014-07-31 Routine/Complete Inspection No Violations Noted The operator has recently installed a casing plug on the well to mitigate stray gas from leaking around the well casing.
2318475 Primary Facility 2014-09-04 Routine/Complete Inspection No Violations Noted Well is being flowed back for production.
2564879 Primary Facility 2017-02-24 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and required documentation was present. After a phone conversation with Chevron it was determined that the Department had issued a major modification approval in October 2016. It is recommended that a new E&S plan and major modification approval letter be place in the mail box. The equipment appeared to be in good condition. The site was well vegetated and Chevron indicated restoration of the site is planned for 2017. There were no signs of erosion observed at the time of inspection nor were violations observed.
