On 04/07/16 at approximately 0840 hours I conducted a routine complete inspection of the Lanco 6-8H well site in response to a restoration extension request submittal by Chevron. The Act 9 sign was in place and all required documents were present in the mail box. The site was stable. The original date for restoration was extended to October of 2014. Due to the extraordinary circumstances resulting from the loss of well control at the site in February of 2014, the Department is approving a new restoration date of October 31st, 2017.

2608629 Primary Facility 2017-06-19 Routine/Partial/Aerial Inspection No Violations Noted An inspection was conducted in response to Gary Bowlen of Chevron requesting a meeting to discuss site restoration. The Act 9 sign was in place and all required documents were present at the site. The site was well vegetated and there were no signs of erosion observed. Site restoration activities had begun. Portions of the access road are to be removed to reduce impermeable surfaces during phase I. Additional portions of the access road are to be removed during phase II. The phase II access road restoration activities have been approved to be completed in phase I.
