2203388 2013-09-06 No Violations Noted On 9/6/2013, I performed an inspection of Chevron Lanco well pad site. The site is located in Dunkard Twp., Greene County. The inspection was performed due to a pre-operation request by the operator.

The new Act 9 Site ID sign was in place and legible.

The well pad, access road and all E&S controls were completed.

A small section of silt sock on the downgradient side of one soil stockpile was filled with sediment. I spoke to the operator about having the area cleaned out and also to watch the area after every precipitation event. I also discussed getting all disturbed areas seeded and mulched.

2225354 2013-11-18 No Violations Noted On 11/18/2013, I performed an inspection of Chevron Lanco well pad site. The site is located in Dunkard Twp., Greene County. The inspection was performed as a routine inspection during drilling.

The new Act 9 Site ID sign was in place and legible.

The well pad, access road and all E&S controls were completed and functional.

All stockpiles and slopes were stable and well vegetated.

Drill rig was on site drilling the 9H-11H wells. All equipment and storage vessels were within containment.
