2472672 Primary Facility 2016-03-21 Administrative/File Review In Compliance with Policy On Monday, March 21, 2016 a brine spill of approximately five to ten gallons occurred on the Porter Street Well Pad located in Franklin Township, Greene County. A leaking valve within a manifold had released the brine directly into containment. The brine was vacuumed out of containment and disposed off-site. The valve was replaced and all lines and tanks were inspected.

The incident has been determined to be In Compliance with Policy (ICP) and no violations are being cited at this time.

2474925 Primary Facility 2016-03-31 Incident- Response to Accident or Event No Violations Noted On March 31, 2016 a release of approximately five to ten barrels of brine occurred at the Porter Street Well Site. The incident was the result of an operator error and had impacted soil on the pad, along the access road and an area on the outer slope of the access road. Response to the incident was immediate upon discovery. The site remediation is being conducted in accordance with Act 2/Land Recycling Program regulations in Chapter 250 of Tile 25 of the Pennsylvania Code.

The failure to contain the brine release is a violation of Title 25 Pa. Code § 91.34(a). The impact to soils by the brine is a violation of the Clean Streams Law 35 P.S. § 691.401. The operator should assess the current design and consider adjustments to prevent a release from bypassing containment in the future. At the time of my site visit on March 31, 2016, Vantage personnel were actively assessing the current design and considering changes to the set-up, as well as changes in protocol and training for contractors.

2497609 Primary Facility 2016-06-08 Compliance Evaluation No Violations Noted On Wednesday June 8, 2016, I conducted a compliance evaluation inspection at the Porter Street Well Site located in Franklin Township, Greene County. The inspection was performed to review remediation work completed in association with a March 31, 2016 brine release in which an estimated three to five barrels of brine was released and had impacted the ground surface.
2553691 Primary Facility 2016-12-27 Routine/Complete Inspection No Violations Noted

On 12/27/16 at approximately 1130 hours I conducted a routine complete inspection of the Porter Street unit. The Act 9 sign was in place and all required documents were present at the site. The production equipment appeared to be in good condition. The site was well vegetated and I observed no signs of erosion at the time of inspection. There were no violations observed at the time of inspection.
