2487344 Primary Facility 2016-05-24 Incident- Response to Accident or Event Violation(s) Noted 5/24/2016, I performed an inspection of the Penneco Braddock 9H well site. The site is located in Aleppo Twp., Greene County. The inspection was performed as a site inspection after a reported spill of crude, brine, diesel and xylene mixture by the operator on 5/20/2016.

The operator informed the DEP that during well service activity the well unloaded through the top of the well on 5/23/16. The well discharged approx. 3-4 barrels of crude/brine/diesel and xylene mixture to the ground around the well head. None of the discharge left the well pad. The service crew shut off the well on 5/24/16 and began removing production equipment for site remediation and cleanup. The operator had Matthew Kapral from Woddard & Curran on site directing site cleanup and removal of impacted material from the pad to be disposed of at an appropriate and department approved location. Impacted material was being stored in three sealed roll off boxes, and also in containment and covered on site. The impacted area around the

2487903 Primary Facility 2016-05-20 Complaint Inspection Pending A spill at Penneco’s Braddock 9H (059-26218) well in Aleppo Township, Greene County occurred Friday May 20, 2016. Reportedly 130 gallons of brine & crude where spilled in addition to 25 gallons of diesel and 25 gallons of xylene. Clean-up has started and Rick Lewis reports a large excavation (300 cubic yards + ?). Turning over to (WQS) 5/24/2016.
2493365 Primary Facility 2016-06-09 Compliance Evaluation No Violations Noted On 6/9/2016, I performed an inspection of Penneco Braddock 9H well pad site with Joel Keller (DEP). The site is located Aleppo Twp., Greene County. The inspection was performed as a compliance evaluation inspection because of violations on 5/24/2016 related to a spill on the well pad.

The operator reported an approx. spill of 130 gallons of crude oil/brine/xylene mixture to the well pad. The operator began clean up activity imediately. The well was placed out of service and production equipment were moved to facilitate remediation of the spill and excavation of impacted areas on the well pad. Impacted material was being stored in three sealed roll off boxes, and also in containment and covered on site. The impacted area around the well head and production lines will remain open and covered until results are received from the confirmatory sampling.
