Multiple violations of 102.5(m)(4) 102.5(c) were observed at the time of inspection and are listed below: -The road to the south of the well pad was not constructed in the approved location -The gravel area to the south of the well pad was not constructed as approved and an increase in impervious area has resulted. -The location approved as an impoundment was constructed as an AST pad and is a significant increase in impervious area. The area between the well pad, gravel pad to the south, and the location that was approved as an impoundment contained a large volume of water.
