2615006 Primary Facility 2017-07-13 Site Restoration No Violations Noted On Thursday, July 13, 2017, I conducted a site restoration inspection at the Koloski Well Pad located in Morris Township, Greene County. The inspection was conducted in response to an operator submittal of a Notice of Termination (NOT) for the ESCGP, ESG13-059-0045, in accordance with 25 Pa. Code Section 102.7.

The inspection of the well site confirmed that all requirements established in 25 Pa. Code Section 102.22(a), regarding permanent stabilization, have been attained.

No violations were identified during the inspection.

I recommend the approval of the NOT request for ESG13-059-0045 which governed earth disturbance activities associated with the Koloski Well Site.

2725555 Primary Facility 2018-04-19 Routine/Complete Inspection No Violations Noted On Thursday, April 19, 2018, I conducted a routine-complete inspection at the Koloski Well Pad located in Morris Township, Greene County.

Gas well 592072 and the associated infrastructure appeared to be in good condition and functioning properly. The well was labelled.

No violations were identified during the inspection.

2811025 Primary Facility 2018-12-04 Routine/Complete Inspection No Violations Noted On Wednesday December 4, 2018, I conducted a routine-complete inspection at the Koloski Well Pad located in Morris Township, Greene County.

The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The gas well was tagged with with identification.

No violations were identified during the inspection.
