2740551 Primary Facility 2018-05-30 Routine/Complete Inspection No Violations Noted On 05/30/18 at approximately 1300 hours I, along with WQS Adam Ofstun and EPS Kevin Maskol, conducted a routine complete inspection of the site.

The drilling rig was being rigged downed and the site prepared for the switch to fracking operations. Sheen was observed on fluids on secondary containment in multiple locations (see photographs). Diesel fuel was observed on secondary containment above the #6 well. A vac truck was mobilized to remove the fluids during the course of inspection

The containment and matting on site appeared to be heavily soiled. Greylock indicated they intended to reuse the secondary containment during fracking operations. It is recommended the containment be thoroughly cleaned, inspected, and repaired as necessary prior to fracking operations.

Soiled equipment used on site was being pressure washed near the edge of containment. The contaminated overspray was reaching the gravel well pad off containment. The containment beneath the truck containing the equipment being pressure washe
