2304784 Primary Facility 2014-07-23 Routine/Complete Inspection No Violations Noted Well not drilled, pre-spud inspection of site.
2483505 Primary Facility 2016-05-04 Incident- Response to Accident or Event In Compliance with Policy An estimated one to two barrels of production water was released from a production tank and directly into containment. The release was the result of a faulty weld and was discovered between stages of fracturing operations. The release was completely contained with no impact to water or soil, the Department was notified according to policy, the response was immediate and the remediation complete. The incident has been determined to be In Compliance with Policy (ICP). No violations are being cited at this time.
2487244 Primary Facility 2016-05-16 Administrative/File Review In Compliance with Policy The separation of a hose from a truck off-loading to an AST through a manifold system resulted in a release of brine at the Good Well Pad on Friday, May 13, 2016. The hose separation resulted from a mechanical failure and released an estimated ½ barrel of brine into containment. Spray from the hose, estimated to be less than a ½ barrel (1/4 to 1/2 barrel) fell beyond containment and impacted gravel on the well pad. The impacted gravel was excavated and disposed off-site. A conductivity probe was used to confirm that all impacted material was removed. The containment was vacuumed out and replaced with new materials. The impacted containment was disposed off-site. Plans are in place to take readings and sample underneath the AST when it is removed to verify that the impact did not extend to or beneath the tank.

The majority of the release was completely contained with no impact to water, the Department was notified according to policy, the response was immediate and the remediation complete. The incid

2823640 Primary Facility 2019-01-04 Routine/Complete Inspection No Violations Noted On Friday January 4, 2019 I conducted a routine-complete inspection at the Good Well Site located in Washington Township, Greene County.

The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The well was tagged with identification.

No violations were identified during the inspection.

2837001 Primary Facility 2019-02-06 Routine/Complete Inspection No Violations Noted On 2/6/2019 at approximately 10:15, I conducted a site restoration inspection of the Good well site located in Washington Township, Greene County to assess site conditions relative to permanent stabilization and permit termination. The following was observed:
  • Act 9 sign located at the site entrance was in good condition.
  • Areas along the access road did not have 70% vegetative cover.
  • Material was observed off the well pad which should be addressed by the operator.
  • Production fluid tanks within secondary containment appeared in good condition.
  • Production equipment appeared in good condition.
  • All wells were marked and appeared in good condition.
  • The site appeared to be stable with no signs of accelerated erosion.

Based on the inspection of the project area, it was determined that all requirements related to permanent stabilization and permit termination had not been attained due to the lack of 70% vegetative cover.

As a result, I do not recommend termination of ESX14-059-0009.
