The site is located in Franklin Twp., Greene County. Present at time of inspection was Noel Messig (SE). The inspection was performed as a routine inspection of site during hydraulic fracturing operations, because of report of a spill by the operator.

The inspection was performed as a compliance evaluation inspection of site during production setup, because of a spill on 9/16/15.
2465827 Primary Facility 2016-03-18 Administrative/File Review In Compliance with Policy An estimated 5 to 10 gallons of brine was released from a breeched T connection within a GPU at the Patton Well Site. The release occurred entirely within containment, therefore no soil or water impacts resulted. The material was removed from containment via a vacuum and in situ testing was conducted to confirm remediation. All associated equipment was replaced and/or repaired. The matter has been determined to be In Compliance with Policy (ICP). No violations are being cited at this time.
2479204 Primary Facility 2016-04-21 Incident- Response to Accident or Event No Violations Noted An estimated 200 gallons of brine was released from a broken site tube on a water truck leaving the Patton Well Site in Franklin Township, Greene County. The release impacted part of the access road between Throckmorton Run and the gate, as well as a few hundred feet of asphalt pavement on Hill School House Road and several areas of compacted gravel berm along the road. Notifications were made, personnel mobilized and the impacted areas remediated.

The failure to contain the brine release is a violation of Title 25 Pa. Code § 91.34(a). The impact to soils resulting from the release of brine is a violation of the Clean Streams Law 35 P.S. § 691.401.

2498660 Primary Facility 2016-06-08 Compliance Evaluation No Violations Noted On Wednesday June 8, 2016, I conducted a compliance evaluation inspection at the Patton Well Site located in Franklin Township, Greene County. The inspection was performed to review remediation work completed in association with an April 21, 2016 brine release in which approximately 200 gallons of brine was released and had impacted the ground surface.

On 10/15/16 at approximately 1200 hours I completed an administrative inspection in response to a release at the Patton pad. A spill out of containment of approximately 1 barrel of water and drilling cuttings was reported, but an accurate volume could not be determined due to rain water being present on the containment. Weavertown Environmental performed the cleanup of the site. SE Technologies LLC submitted a letter on Vantage’s behalf requesting that the Act 2 process be waived, due to the inconclusive volume released and low conductivity levels. The Department determined that the evidence provided was reasonable and has permitted Vantage to continue without the Act 2 process. The spill was handled in compliance with DEP policy.

2550436 Primary Facility 2016-12-27 Routine/Complete Inspection No Violations Noted

On 12/27/16 at approximately 1030 hours I conducted a routine complete inspection of the Patton Unit. The Act 9 sign was in place and all required documents were present at the site.

A U shape of sediment sock had been placed immediately adjacent to the first stream crossing to serve as an infiltration berm for water eroding the road surface. It is highly recommended that the road be repaired and the material stopped by the sock removed to prevent a BMP failure at the stream.

The GPUs were found to be in secondary containment consistent with Chapter 78A.64b which requires equipment containing regulated substances to be placed within secondary containment.

The site overall was well vegetated and no signs of erosion were observed at the time of inspection with the exception of the road surface.
