What appears to be a bucket is located around the monument. According to records, well drilled 6/7/1965-6/27/1965. Notice of Intent to Plug received by the Department on 5/9/2016.

Site ID sign present. Site has been re-graded, seeded, and mulched and is nearing 70% uniform and adequate vegetative growth. Silt fence installed along site perimeter. Access road not yet restored as rocked access road entrance remains. Per current regulations, within 9 months after plugging a well, the Operator shall remove all production or storage facilities, supplies and equipment and restore the well site. What appears to be a hand-drawn E&S map is located on-site. The Department was unable to locate an adequate on-site E&S plan containing all of the required plans and narratives. The Department’s regulations provide that the E&S plan shall be present at the project site during all stages of an earth di

2565039 Primary Facility 2017-02-21 Administrative/File Review Outstanding Violations - Viols Req'd Admin inspection in response to inspection ID 2542514 (enf ID 349260) for failure to design an E&S plan that contains all of the required plans and narratives and for failure to have an E&S plan on-site for all stages of an earth disturbance activity.

Operator written response to NOV was received on 2/17/2017. Per the Operator’s response, a complete E&S plan is now available at the site. According to this response, the access road is to remain for the landowner’s benefit. The Department recommends the Operator provide documentation from the landowner confirming that the access road is to remain. A follow-up inspection will be conducted. Violations remain outstanding.
