Inspt in response to admin inspt 2270433.Upon notification of spill from answering services, Dept contacted XTO rep E.Greenert via phone.Mr. Greenert stated a release of approx 1 bbl brine into an earthen dike occurred as a result of a faulty phalange on a prod tank.Roll-offs currently en route to site, E&S BMPs to be installed.Water truck already removed remaining contents from tank.Impacted areas delineated.It is not believed that any waterways have been impacted although a stream is located approx 200 ft away from the well.Oriskany. According to records, drilling occurred 9/14/1978-10/4/1978. Well tag present, legible.

2405243 Primary Facility 2015-08-25 Compliance Evaluation No Violations Noted On-site CEI in response to inspection record 2270478 (enf 310972) for the 5/6/14 reportable release of 1 bbl brine water.Area of excavation partially backfilled. Backfilled areas seeded/mulched with moderate growth observed.Silt sock installed down-gradient of excavated areas.Per the spill policy, 15 day initial report received by Department on 5/23/14 notifying Department that Operator is utilizing Act 2 for site remediation. NIR received by Department on 11/7/14.As the Operator has entered the Act 2 program and remediation is ongoing, violations issued under inspection record 2270478 (enf ID 310972) have been corrected.While on-site, Department contacted Operator representative Jason Canose via phone to inquire about status of remediation (i.e. backfilling). Per Mr. Canose, the backfilling of the site is to be completed as soon as possible. Mr. Canose agreed to provide the Department with updates as appropriate. UPDATE: At 1338, Mr. Canose notified the Department via phone that backfilling is to be completed next week.

2446229 Primary Facility 2016-01-27 Routine/Partial Inspection No Violations Noted Oriskany. At time of inspection, well appears plugged with erected monument. Name plate with permit number attached to erected monument. According to records, drilling occurred 9/14/1978-10/4/1978. Notice of Intent to Plug received by Department on 9/30/2015.

E&S plan on-site. Orange fence located around wetland identified on E&S plan. Temporary BMPs appear to be installed per the E&S plan, however Department unable to adequately inspect these BMPs due to snow-cover. Although site is snow-covered, well site does not appear to yet be re-graded and/or restored in accordance with the restoration plan. Department unable to adequately inspect site and access road for any accelerated erosion and sedimentation issues during this inspection due to snow-cover.

2536016 Primary Facility 2016-11-07 Routine/Complete Inspection No Violations Noted At time of inspection access road gate remains at beginning of access road. Beginning section of access road rocked, the remainder of the access road appears to have been removed and permanently restored. Department contacted XTO Rep Dan Rice to determine if gate and a portion of access road are to remain. Mr. Rice stated gate and beginning of access road are to remain to allow access to pipeline.

Well appears to be plugged. Permit number affixed to erected monument in a permanent manner. Site appears permanently stabilized with >70% uniform vegetative growth. Meter box remains on site. Operator Rep Dan Rice stated meter box will be removed this week. UPDATE 11-10-16: Operator Rep Dan Rice contacted Department by phone and left message stating meter box has been removed from site.

2785547 Primary Facility 2018-10-09 Site Restoration No Violations Noted Inspection conducted in response to a Well Site Restoration Report received by Department. As per inspection record #2536016, gate at entrance of access road remains and meter box has been removed. At time of inspection well is plugged. Site vegetated and appears returned to near original contours.

This restoration report is approved.
