13:15. Department contacted operator by phone. Left message. 13:18. Operator rep Lisa Raffle contacted Department by phone. Ms. Raffle stated a release of approximately four barrels of brine/crude oil occurred approximately 11:30 today. Ms. Raffle stated she feels the brine tank was compromised by animal activity. She also stated brine tank is in an unlined earthen dike and waterways were not affected by this release. Ms. Raffle stated water truck would be on site to empty brine tank today. 13:52. On site inspection. At time of inspection water truck has just left site. Brine tank located in an earthen dike (photo #1). Brine tank remains on site and connected to production equipment. No fluids observed in the dike. No moist soil or wet rocks observed. It appears the released fluids entered the ground under the brine tank. Department recommends operator remediate site per Document #800-5000-001: Addressing

2726793 Primary Facility 2018-05-03 Administrative/File Review Outstanding Violations - Viols Req'd ADMIN inspection to inspection record #2724416. Operator rep Lisa Raffle contacted Department by phone. Ms. Raffle stated their lease operator made an error and the previously reported release at this site never occurred. Ms. Raffle requested a site meeting with Department to inspect the site and obtain soil probe readings.

Department requested tank readings showing where error occurred and would like to inspect bottom of brine tank to ensure tank is structurally sound. A site meeting is tentatively scheduled for Thursday May 10th. Violation issued on 4-25-18 remains outstanding.

2730295 Primary Facility 2018-05-10 Follow-up Inspection Outstanding Violations - Viols Req'd Follow up inspection to inspection record #2724416 where a violation was issued for an operator reported release of 4 bbl of brine/crude oil.

On-site inspection requested by operator. Present during inspection are operator representatives Lisa Raffle, Gordon Bish, and Dusty Duckworth. At time of inspection, brine tank is empty, unattached to production equipment, and moved to southern end of site. As per Ms. Raffle, the lease operator made an error and mistakenly reported a 4 bbl release of brine/crude oil. Operator is unable to produce documentation supporting this statement. Department collected soil probe readings from area where brine tank was installed when release was reported. No elevated readings were observed. The area was rocked and the entire area was not sampled. Department recommends operator allow Department access to bottom of brine tank to assess structural integrity, or professionally sample soil to ascertain if a release of brine/crude oil occurred. 2730760 Primary Facility 2018-05-16 Compliance Evaluation No Violations Noted Compliance Evaluation inspection to inspection record #2724416 where a violation was issued for an operator reported release of 4 bbl of brine/crude oil.

On-site inspection requested by operator to allow Department to visually inspect bottom of brine tank. Present during inspection are operator representatives Lisa Raffle, Dusty Duckworth and 2 contractors. At time of inspection, brine tank is empty and tipped on its side so bottom of tank is visible. Department is able to visually inspect bottom of tank and does not observe any cracks or holes on the tank. Due to the brine tank appearing structurally sound, no elevated readings when testing soil for brine (inspection record #2730295), and no visual evidence of a release (inspection record #2724416), it appears that the operator reported release of 4 bbls of brine/crude oil did not occur. Violation issued on 4-25-18 is closed.
