2396590 Primary Facility 2015-08-03 Administrative/File Review In Compliance with Policy Admin inspt in response to release reported under record 2396559.Upon receipt of spill report, Dept contacted Opr repDrew Underwood via phone at 0835.Per Mr. Underwood, landowner notified Opr on 7/31 at 1030 of damage to BT w/brown grass behind/surrounding BT.Release was subsequently reported to SWRO by Mr. Underwood at 1103.Upon further investigation, Opr determined that “the dump line on the separator was knocked off” and there was a “gash on the bottom of the fiberglass brine tank” which is believed to have resulted in the 13.5 bbl brine release to ground.Mr. Underwood believes that an ATV crashed into prod equipment as there are reported tracks off the AR that lead to the equipment, w/mud also splattered on the equipment.Mr. Underwood reports that pieces of a headlight,hubcap were discovered at site. Mr. Underwood estimates release to be 13.5 bbl as BT was empty at time of investigation and last reading, which occurred on 7/13/15, had fluid quantity measured at 13.5 bbl.
2396796 Primary Facility 2015-08-03 Incident- Response to Accident or Event Violation(s) Noted On-site inspt conducted in response to admin inspt 2396559 for the reportable release of 13.5 bbl brine water.At time of inspt, excavation of contaminated materials in process, currently awaiting arrival of 2 add’l roll-off boxes.Two covered roll-off boxes on-site.Visibly impacted materials appear to be located within an approx area of 25’x30’.Temp BMPs installed around the perimeter of excavation.Opr rep Jim Zaczyk on-site.Dept recommended that, in interim, the area of excavation is covered with plastic and/or a tarp in order to prevent any further migration of contaminants should any precipitation events occur.Excavation is anticipated to be completed today, tomorrow at the latest.No nearby waterways appear to be impacted.Result of release is believed by Opr to have occurred due to ATV crashing into BT and separator.Mr. Zaczyk stated that pipes connected to the separator were disconnected and believes this resulted during the collision. Viols are being issued for reportable release of production fluids.
2417288 Primary Facility 2015-08-03 Routine/Complete Inspection No Violations Noted OGI performed inspection of well Irwin 2 well 063 – 24922 and met with Jim Zaczyk and Ken Hill from XTO at the site. Reviewed the accident at well head. It is assumed that an ATV traveling at a high rate of speed in the westerly direction hit the brine tank which was holding approximately 13 barrels or about 4000 pounds. The collision caused a tear in the base of the tank resulting in a release of fluid to the flat area of ground north of the tank. The collision also caused the tank to move from its original location. At the time of incident, the well was inoperable due to ongoing repair work to the lateral pipeline from the well to the main supply line. The well was locked and tagged out and still remains. The collision did not damage nor disturb the gas wellhead or remaining production equipment. XTO predicted accident occurred about week ago based on vegetation and spill release area.

2420575 Primary Facility 2015-10-29 Compliance Evaluation No Violations Noted Admin inspt in response to viols issued under record 2396796 (enf ID 328458) for reportable release of 13.5 bbl brine water to the ground.In accordance with the Spill Policy, the 15 day initial written report was received on 8/17/2015 notifying the Department that the Operator will be utilizing Act 2 for the remediation of this release.Per a 1442 phone call with ECB Compliance Specialist Evan Elliot, the Notice of Intent to Remediate was received by the Department on 10/21/2015.As the Operator has initiated remediation (inspection ID 2396796) and has identified their intent to remediate under the Act 2 program, violations issued under inspection record 2396796 (enf ID 328458) have been corrected.A follow-up inspection is anticipated to be conducted in the future to ensure site is properly restored as per current regulations. Violations may be forthcoming pending future inspections monitoring site restoration and permanent stabilization.
