2406226 Primary Facility 2015-09-02 Administrative/File Review No Violations Noted Brad Little called at 8:50am in response to Department call on 9/1/2015 in regards to observations of a potential release. He informed Department of what he believes is a release of approximately 50 gallons of an unknown material. Department recommends Operator identify source and type of material released and perform clean-up in accordance with Spill Policy. Operator shall follow-up with Department OGI in regards to any wellhead integrity issues that may be related to this release. Violations may be forthcoming pending further investigation and review of additional information provided by the Operator. Department will conduct a follow-up inspection.
2406999 Primary Facility 2015-09-03 Routine/Partial Inspection Violation(s) Noted RTPT inspection conducted in response to inspections IDs 2406224 & 2406226. At time of inspection, well drilled and appears equipped for production. Opr repBrad Little and contractors on-site. Contractors currently plumbing the 2” annular port into the brine tank in response to fluid discharge from this port. Mr. Little believes that “formation water” is leaking out of this port. Mr. Little stated that he was unaware of this issue and is therefore unaware of how long it has been occurring. Mr. Little reports that the water comes out of the port as a “spit” and/or “trickle”.Due to the nature of the discharge (i.e. trickle), the Department was unable to collect an adequate quantity of sample material for standard oil and gas analysis. Mr. Little stated that he intends to collect a sample of the fluid.The Dept informed Mr. Little that a FUI is anticipated to be conducted to observe whether the Opr has properly identified and remediated the discharge.

2420026 Primary Facility 2015-10-02 Routine/Complete Inspection No Violations Noted DEP met with Brad Little and his consultant relative to this well. As you recall, this well had water leaking out of the annulus. Penn View dug about 3 tons of soil and plumbed annulus into the brine tank. We reviewed the geophysics log and well completion report. Surface casing was set at 1330 feet and cemented to surface. Production casing was set at 3300 feet. Top of cement on the production casing was around 1900 feet based on geophysics log. Penn View plumbed the annulus to the brine tank and monitoring the amount of water flow from plumbing.
2447939 Primary Facility 2016-02-04 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspection conducted in response to violations issued under inspection ID 2406999 (enf ID 329689) for the reportable release of pollutional fluids to the ground.

Operator previously submitted an Investigation and Report of Small Spill Incident to the Department. This report was reviewed by a Department Soil Scientist. At 1447, the Department left a message for Operator representative Brad Little via phone. At 1454, Mr. Little returned the Department’s phone call. The Department recommended to Mr. Little that additional excavation is performed in the vicinity of those soil samples exceeding standards for chlorides (i.e. samples 2, 4, 6). The Department recommended that this additional excavation is conducted as soon as possible and that temporary E&S BMPs are installed as necessary. Mr. Little agreed to promptly implement the Department’s recommendations. The Department also requested that all disposal records/manifests are submitted to the Department upon final disposition of wastes.

2450704 Primary Facility 2016-02-16 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspection conducted in response to violations issued under inspection ID 2406999 (enf ID 329689) for the reportable release of pollutional fluids to the ground.

At 1522, Operator consultant Van Plocus notified the Department via phone that additional excavation and sampling had been performed as recommended in inspection ID 2447939.

2454837 Primary Facility 2016-02-25 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspection conducted in response to violations issued under inspection ID 2406999 (enf ID 329689) for the reportable release of pollutional fluids to the ground.

At 1057, Operator consultant Van Plocus notified the Department via phone that the additional soil sample results have been received (inspection ID 2450704) and supplemental documentation will provided to the Department detailing these results. Disposal manifests for any wastes generated during the remediation of this release will also be provided to the Department.

2463275 Primary Facility 2016-03-10 Compliance Evaluation In Compliance with Policy On-site CEI conducted in response to violations issued under inspection ID 2406999 (enf ID 329689) for the reportable release of pollutional fluids to the ground.

At time of inspection, there is an area of excavation surrounding the wellhead that is not yet backfilled. Excavated soils on-site, appear to be contained and covered with plastic liners. Operator previously submitted an Investigation and Report of Small Spill Incident to the Department and is currently implementing recommendations made by the Department (inspection IDs 2447939, 2450704, 2454837) in response to this report in order to ensure that all contaminants have been adequately excavated. As Operator appears to be in compliance with policy at this time insofar as the Operator has identified their intent for remediation and has submitted an investigation report as noted above, violations issued under inspection ID 2406999 (enf ID 329689) have been corrected.
