No E&S issues observed on access road or on site. Well tag becoming illegible. Department attempted to contact Operator at phone number listed in Department records but number was no longer in service. Department recommends Operator update contact information to reflect Operator's current phone number. Department also recommends Operator replace well tag to ensure legibiltiy. No brine tank on site. Slight gas odor and bubbling sound coming from well head area. A follow up inspection will be conducted at a later date to ensure well tag is legible.

2725300 Primary Facility 2018-05-01 Routine/Partial Inspection Violation(s) Noted Routine Partial Inspection conducted in response to a referral received by DEP OGI Jason Branan on 5-1-18.

Well site located in a clearing; no brine tank on site (photo #1). At time of inspection it appears fluid or some substance may have sprayed from well and coated nearby vegetation/ground (photo #2). No active spraying or release is observed at this time. The impacted vegetation/ground is on all sides of the well head (photo #3) and extends ~30 feet in some directions. The substance coating the vegetation/ground appears brown and does not have an odor or sheen or oily texture. In addition, well is making a hissing/bubbling noise and the well head occasionally shifts/moves. Department collected soil samples to determine nature of fluid/substance released. This inspection is PENDING results of soil sample analysis. A follow up inspection will be conducted at a later date. Violation(s) may be issued based on follow up inspection(s). UPDATE 5-18-18: Department received and reviewed soil analytical result
